Overview:

Community Care Coordinator (LPN or CNA)

 

Full-Time, Daylight shift; 7:00 a.m. to 3:00 p.m., every other weekend

 

Located in Center Twp, Beaver County

 

Lutheran SeniorLife provides a comprehensive array of community based health and social service programs throughout western Pennsylvania.

 

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

 

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

 

The Community Care Coordinator CNA or LPN will enhances the abundant life of participants by providing assistance through performance of routine direct and indirect participant activities according to established policies and procedures. Maintains participant rights, privacy, and confidentiality. Promotes participant acceptance of care plans and responds promptly to overall participant needs.

Qualifications:

Community Care Coordinator (LPN or CNA)

  • High school diploma or general education degree (GED); or 1 year related experience and/or training; or equivalent combination of education and experience.
  • LPN or CNA
  • Must have one (1) year of geriatric related experience;
  • Working knowledge of computers, Microsoft Word and Excel required;
  • Excellent interpersonal skills, team-oriented, knowledge of medical terminology, computer literate.
  • Must have and present current valid driver's license.
  • CPR and Certified First Aide required.
