THIS IS A 24 HOURS PER WEEK BENEFIT ELIGIBLE POSITION AVAILABLE AFTER JANUARY 1, 2026. 
THE ANTICIPATED SCHEDULE IS MONDAY THROUGH THURSDAY FROM 9 AM - 3 PM. 
THE PAY AS SHOWN INCLUDES THE 2% COST OF LIVING ADJUSTMENT IN EARLY 2026.
 

About us
The City of Fitchburg is searching for another Case Manager to join our fun team!  The mission of the Fitchburg Senior Center is to advance healthy aging from vibrant to vulnerable populations through diverse social, recreational, and volunteer opportunities and supportive services.

How you will make a difference
Under the general direction of the Recreation & Community Services Director, you will advocate for, assess, mediate, and support the community of older adults and their caregiver network.  You will provide case management coordination and record-keeping for individuals with multiple needs. Decisions within areas of responsibility are made independently.  There is frequent interaction with older adults, their families, various health care providers, other city departments, and outside social service agencies.  Must be able to work with culturally diverse groups and individuals and ensure a welcoming environment for all.  

Position Duties

Essential Functions for Senior Center

  • Assess and interpret needs of Fitchburg residents over 50 years of age.
  • Develop client centered service plans with the input of the client.  
  • Provide case management coordination and record keeping for individuals who have multiple needs regardless of income status.
  • Maintain client files with accurate up-to-date intake, assessment and case notes.
  • Follow the Case Management standards as defined by the Area Agency on Aging Board. 
  • Offer information and assistance as needed to those individuals who do not meet the criteria of case management client.
  • Refer clients to the Adult and Disability Resource Center for enrollment in Family Care.
  • Make referrals to Dane County social service agencies as needed; follow up as necessary.
  • Complete required monthly reports and submit them in a timely manner.
  • Meet or exceed requirements of the Dane County contract which include unduplicated client count and service hours.
  • Treat clients with dignity and respect in a fair and equitable manner.
  • Exercise professional judgment, ethics and maintain confidentiality.
  • Participate in on-going training at state, county and local level. 
 
Additional Duties and Responsibilities:
  • Participate in regular staff meetings. 
  • Provide community outreach and education.
  • Mentor social work interns when needed.
  • Assist with special events when needed.
  • Other duties as assigned.

Minimum Qualifications

Supervisory Requirements:
•    None

Minimum Qualifications
Education and/or Experience Requirements:
•    Bachelor’s Degree in Social Work, Human Services or related field required.  
•    Prior social work experience desired.
•    Knowledge of Dane County and other social service providers desired.
•    Ability to speak Spanish desired.

Licenses, Certifications:
•    Valid Wisconsin Driver’s License.

Necessary Knowledge Skills and Abilities:

  • Ability to build and nurture relationships and build a welcoming atmosphere.
  • Ability to build effective working relationships with City Departments, clients, volunteers, outside agencies and community members.
  • Ability to work effectively with culturally diverse individuals and groups. 
  • Ability to adapt to changing work priorities.
  • Ability to handle participants/clients that have a history of mental health, AODA issues, memory loss and/or other health issues.
  • Ability to perform detailed work with great accuracy required.
  • Ability to work independently with only occasional review of work performance.
  • Ability to utilize computer technology to access, retrieve, or input information.
  • Advanced knowledge in the utilization of computer technology including word processing, spreadsheet, relational databases, and other related software programs.  Preference given to those who know Excel, Word, Access, Publisher and Powerpoint.
  • Ability to communicate effectively both verbally and in writing with a variety of people, including co-workers, other professionals, elected officials, and the public.
  • Proficiency in word processing/typing.
  • Ability to foster cooperation and teamwork among peers.
  • Ability to maintain adequate attendance. 
  • Ability to establish and maintain effective working relationships with City elected and appointed officials, employees, and the general public.
  • Ability to attend meetings and other functions during the evening and/or on weekends.
