Job Summary
Partners in Home Care is seeking a compassionate and skilled Registered Nurse (RN) – Case Manager to join our team. This position is responsible for delivering high-quality nursing care and managing the overall coordination of home-based services for assigned patients. Using an interdisciplinary team approach, the RN will assess, plan, implement, and evaluate individualized care, ensuring continuity, quality outcomes, and optimal patient and family support.
This position may begin as a Visiting RN, transitioning into a Case Manager role once a half caseload is assumed, in accordance with the Collective Bargaining Agreement with Montana Nurses Association Local #32. Both roles share the same clinical responsibilities.
Essential Duties and Responsibilities
- Provide skilled nursing care and coordinate services for patients in their homes based on comprehensive assessments and physician-authorized care plans.
- Continuously observe, assess, and evaluate patient conditions—physical and psychosocial—and adjust care interventions accordingly.
- Supervise and evaluate care provided by Home Health Aides and CNAs, ensuring adherence to the patient’s care plan.
- Maintain open and timely communication with physicians, interdisciplinary team members, and external community resources to ensure continuity of care.
- Educate and support patients and families through counseling, symptom management, health teaching, and referrals, promoting quality of life and optimal outcomes.
- Accurately document all patient care in accordance with agency standards and regulatory requirements (OASIS/HIS/HOPE), with working knowledge of current reimbursement rules and compliance guidelines.
- Participate in interdisciplinary scheduling and rotate into the evening/weekend on-call schedule, assessing and addressing patient needs via phone triage or in-person visits as needed.
- Engage in agency quality improvement activities and assume responsibility for achieving relevant quality metrics. Attend required in-service training sessions.
- Meet caseload and productivity expectations aligned with agency standards.
- Perform other duties as assigned by supervisor.
Minimum Qualifications
- Current Registered Nurse (RN) license in the State of Montana (BSN preferred).
- Valid driver’s license, auto insurance, and access to reliable transportation.
- Preferred: Two years of recent clinical experience in acute care, home health, or hospice, including skills such as venipuncture, catheter care, wound management, and parenteral therapy.
Schedule
Typical workdays are Monday through Friday, with evening/weekend on-call rotation. Schedule may vary depending on FTE status and patient care needs, as directed by the supervisor.
