Description

SUMMARYThe Integrated Clinical Specialist – Mental Health is the lead clinician in the delivery of services to patients diagnosed with substance use disorders and serious and persistent mental illness.  This position is responsible for the provision of clinical services including assessment and diagnosis, treatment planning, mental health and addiction therapies, family services, consultation, case management and coordination with community and medical service partners.

 

 

ESSENTIAL RESPONSIBILITIES AND TASKS: 

 

  • Evaluate patients thoroughly to identify treatment needs and to direct holistic treatment approach. Conduct screening and assessment activities to determine detoxification, substance use and mental health diagnosis, level of care and functioning, and recommend a clinical course of treatment.
  • Apply motivational techniques and case management to promote engagement in treatment.
  • Develop and maintain a collaborative working relationship with all clinical and management team membersCreate solutions that best fit a patient’s needs, determining appropriate delegation of duties to team members.
  • Provide on-going risk assessment and take appropriate steps to mitigate risk
  • Provide engaging patient education, cognitive behavioral, psycho-education, process, and relapse prevention group therapies.
  • Act as liaison and bridge to family, community involved agencies, primary care, and other service providers
  • Provide services that are family and community oriented, person centered, culturally competent, and trauma-informed.
  • Provide case management, resource information and referral services.
  • Provide assertive coordination of care with adjunct medical and other service providers.
  • Assist Residential Administrator (RA) in liaison work with the area and state hospitals, and oversee evaluation and transition plans for potential program patients.
  • Participate in on-call coverage and crisis evaluation within Director Designee responsibilities
  • Maintain positive working relationships with county, state, AMHI, and community service providers.  Attend community meetings as necessary.
  • Provide timely, accurate and complete medical record documentation.
  • Utilize time efficiently to maximize patient contact hours and ensure assigned patients receive the services prescribed by their treatment plan and are appropriate for their diagnosis and level of care. 
  • Actively support the treatment team approach to patient care, participating in staff meetings, case presentations, clinical consultations, and operational process review. 
  • Demonstrate effective clinical judgment to support safe and quality treatment.
  • Maintain credentialing and adherence to ethical guidelines.
  • Travel to community-based meetings and across sites for patient support at other locations.
  • Consistent attendance and punctuality are required of this position.

SECONDARY RESPONSIBILITIES AND TASKS: 

  • Provide comprehensive team coverage during team absences or assisting with a shared team responsibility.
  • Participate and engage in clinical supervision and professional development activities.
  • Provide toxicology testing as needed.

Qualifications


REQUIRED EDUCATION AND EXPERIENCE:

  • Master’s degree in Social Work, Counseling, or Psychology
  • Minimum 2 years of experience working in community or behavioral health system providing clinical services to individuals who have co-occurring mental health and substance use disorders.

 

COMPETENCIES AND SKILLS:

  • Has advanced knowledge of and is able to effectively apply advanced principles of motivational interviewing, cognitive behavioral therapy, trauma informed care, and similar strategies for facilitating recovery and empowerment.
  • Has an advanced understanding of and the ability to recognize and effectively apply mental status concepts, mental health symptomology, and commonly used mental health treatment and service terminology.
  • Is able to accurately utilize established criteria for assessing and assigning mental health and substance use disorder diagnoses.
  • Ability to develop a coordinated treatment plan consistent with the patient’s substance use and mental health diagnoses, needs, preferences, and placement criteria; ability to identify and actively collaborate with appropriate other parties for treatment coordination.
  • Ability to implement and coordinate the services and supports identified in a treatment plan for a dually diagnosed individual, including effectively communicating with and engaging patients in their treatment plans and applying interventions as needed to increase engagement.
  • Ability to recognize, take preventative measures, de-escalate, and appropriately respond to individuals in crisis and crisis situations in the treatment environment, including non-physical interventions, risk assessments, safety plans, and the utilization of additional resources as appropriate.
  • Ability to utilize both oral and written communications skills to effectively communicate with patients, co-workers, community resources, supervisors, and various other individuals encountered in the course of performing work while maintaining patient confidentiality and establishing professional rapport.
  • Ability to operate a computer, utilize basic MS Office programs, and effectively navigate and maintain patient records in an electronic medical records system utilizing concurrent documentation techniques, clinically accurate terminology, and standardized documentation methods.  

 

REQUIRED CERTIFICATIONS AND LICENSES:

  • Qualified Mental Health Professional
  • May be required to attain a Basic First Aid/CPR certification or ability to acquire within 90 days of hire.
  • Current Oregon or Washington State Driver’s License.

 

PHYSICAL DEMANDS: 

  • Ability to safely lift up to 25 pounds.
  • Ability to safely utilize Personal Protective Equipment in working with biohazard and Blood Borne Pathogen exposure.
  • Must be able to follow proper protocol while interacting directly with patients and working in a clinical environment.
  • Ability to safely operate a motor vehicle.
  • Ability to safely utilize a computer for extended periods of time, including keyboarding, using a mouse, reading information from a monitor, and sitting at a workstation.
