Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to the Clinical Quality Integration Team. The Community Health Float RN will provide essential clinical support to patient care activities utilizing nursing education, assessment and procedural skills at all Erie sites as needed.

*Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
Main Duties & Responsibilities
- Collaborates with providers in the delivery of patient care
- Participate in the development and implementation of individual, group and community education programs. Triage assessment process of walk-in patients and telephone calls.
- Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
- Provide training to new nurses in areas of specialty and strengths.
- Collaborates with appropriate members of other divisions to ensure optimal coordination of care for the patient.
- Able to commute to all sites, including Erie Waukegan, as needed
- Be available at times determined by parity and greatest need, including Saturdays and closing, unless days off are approved in advance.
Qualifications
Education:
- Current Active Illinois RN Licensure
- Current CPR Certification
Skills and Knowledge Required:
- One (1) year of experience in community health nursing or ambulatory care required
Preferred:
- Proficiency in Spanish is strongly preferred
- Intermediate proficiency in Microsoft Office a plus
- Intermediate EPIC skills are a plus
- BSN Preferred
Education:
