Ampla Health is seeking a compassionate and organized HOPWA Case Manager to support individuals affected by HIV/AIDS. Under the direction of the HIV Program Manager, this role provides intensive case management, housing support, and crisis intervention to promote health stability and prevent homelessness.

Key Responsibilities:

  • Conduct intake assessments to determine eligibility and client needs.

  • Develop and update individualized service plans using trauma-informed and motivational interviewing approaches.

  • Provide housing case management, advocacy, and referrals for benefits and services.

  • Coordinate with medical professionals, outside agencies, and Ryan White Case Managers to ensure continuity of care.

  • Track and maintain documentation in compliance with grant and program requirements.

  • Submit invoices, assist with audits, and support program reporting.

  • Participate in case conferences, training, and continuous quality improvement (CQI) efforts.

Qualifications:

  • High School Diploma required; Medical Assistant certification required within 90 days of hire.

  • Strong interpersonal, communication, and problem-solving skills.

  • Proficient in Microsoft Office; able to type at least 40 wpm.

  • Experience in case management, housing support, or HIV-related services preferred.

Physical Requirements:

  • Ability to lift up to 40 lbs. and perform standard office tasks.
