About the Role:

Job Summary

The primary responsibilities for Behavioral Health Specialist will be to provide health consultation to adults and families in order to improve psychosocial functioning and to collaborate with an interdisciplinary care team to develop a patient centered plan of care that will improve outcomes, decrease cost and improve the patient's experience. When social determinants of health are a factor, the Behavioral Health Specialist may address these through connections to community resources and services.
 

Minimum Job Qualifications

  • Licensure or other certifications: Georgia license in Social Work (LCSW) or Licensed Professional Counselor (LPC) required. Maintains current BCLS certification. 

  • Educational Requirements: Masters Degree; in Social Work from a school accredited by the Council on Social Work Education (CSWE) or Masters Degree in Counseling/Psychology from a school accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP), the Council on Rehabilitation Education (CORE), or a regionally accredited program recognized by the Council on Higher Education Accreditation (CHEA).

  • Minimum Experience: Three (3) years experience in a hospital, agency or institution providing related health care services. Must be able to demonstrate a base of knowledge related to behavioral medicine and evidence-based treatments for medical and mental health conditions and have a thorough understanding of psycho-pharmacology.

Essential Tasks and Responsibilities

  • Promote the delivery of high quality service to our patients through comprehensive assessment and diagnosis of behavioral health needs.

  • Provide effective treatment planning and assistance for patients to help them successfully achieve their goals.

  • Evaluate psychiatric related crisis situations and apply appropriate interventions.

  • Actively participate as a member of an interdisciplinary team to support the integrated health care model to provide comprehensive care for clients.

  • Assist in the detection of "at risk" patients and in the development of strategic approaches designed to prevent further psychological or physical deterioration.

  • Assist the primary care physician and care team to develop care management processes such as the use of guidelines, disease management techniques, case management and patient education to improve self-management of chronic disease.

  • Provide assessment, consultation and brief intervention for psychological/psychiatric problems and/or disorders.

  • Teach patients, families and members of the care team techniques that can be used to improved success with behavior modification.

  • Monitor key indicators of the success of the behavioral health program, identifying opportunities and making suggestions for improvement.
