About The Night Ministry

The Night Ministry is a Chicago-based nonprofit that provides housing, health care, and human connection to individuals experiencing homelessness or poverty. Our Youth Flexible Housing Pool (YFHP) program offers permanent supportive housing for youth ages 18–28 who are experiencing or at risk of homelessness. By working collaboratively with partner agencies, we help young adults stabilize, thrive, and transition into independence.

As a Housing Case Manager with the YFHP team, you'll work primarily in the field, supporting young adults through housing transitions, service coordination, and lease compliance. Your work will be rooted in harm reduction, trauma-informed care, and positive youth development.


What You'll Do

You'll be responsible for providing intensive case management to a caseload of youth participants-ensuring timely support as they move into and maintain permanent housing, and helping them develop life skills that support long-term independence.

Key responsibilities include:

  • Conducting home visits and assessments; developing individualized case plans.
  • Supporting lease-up, housing retention, and crisis intervention needs.
  • Coordinating move-ins, furniture, starter kits, and transition planning.
  • Advocating for participants with landlords and subsidy partners (e.g., Center for Housing and Health).
  • Supporting participants in maintaining lease compliance and addressing eviction risks.
  • Connecting participants to benefits such as SNAP, SOAR, Medicaid, employment services, and behavioral health care.
  • Providing on-call case management on a rotating basis with the housing team.
  • Documenting all services and activities in ETO and CaseWorthy databases.
  • Collaborating with internal and external housing partners, outreach teams, and employment case managers.
  • Ensuring trauma-informed, youth-centered care is integrated throughout your work.

What We're Looking For

You're a resourceful, compassionate, and detail-oriented professional with a passion for youth empowerment and housing justice. You thrive in field-based roles, understand harm reduction and housing-first models, and are confident navigating complex systems.

Qualifications:

  • Bachelor's degree in social work, counseling, or related field or significant equivalent experience.
  • Minimum of 3 years of experience working with at-risk youth.
  • Experience providing home-based or field-based services in a housing context.
  • Valid driver's license, reliable vehicle, and clean driving record.
  • Familiarity with case documentation systems and community-based resources.
  • Strong organizational, communication, and crisis management skills.
  • Ability to work flexible hours, including evenings, early mornings, and occasional weekends.
  • Clinical or behavioral health experience is a plus.
  • Commitment to harm reduction, housing-first practices, and trauma-informed care.
