Primary Duties and Responsibilities:

  • Support the Patient Coordination team in conducting outreach calls to patients post-discharge, ensuring follow-up appointments, medication adherence, and patient understanding of discharge instructions.
  • Assist patients with non-clinical needs, including navigation of healthcare resources and referrals.
  • Collect and accurately document patient information in designated systems.
  • Monitor and record patient-reported concerns, escalating clinical issues to licensed staff as appropriate.
  • Provide compassionate communication and patient education under supervision, reinforcing instructions related to care plans and self-management.
  • Assist with administrative tasks, including organizing records, maintaining data accuracy, and supporting patient scheduling efforts.
  • Collaborate with healthcare team members to ensure timely resolution of patient needs and service coordination.
  • Adhere to organizational policies, patient confidentiality requirements, and compliance protocols.

Knowledge, Skills, and Abilities:

  • Strong interpersonal and communication skills with a patient-centered approach.
  • Basic understanding of anatomy, physiology, medical terminology, and patient care processes.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Demonstrated organizational and documentation skills.
  • Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and collaboration platforms (Teams, Webex).
  • Compassion, empathy, and a commitment to providing quality patient interactions.

EDUCATION (Required):

  • Currently enrolled in an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program.

QUALIFICATIONS:

Previous Experience:

  • Completion of at least two semesters or one clinical rotation.
  • Customer service or healthcare-related experience is a plus.

Knowledge, Skills, and Abilities:

  • Ability to demonstrate professionalism, reliability, and a strong sense of accountability.
  • Strong interpersonal skills with the ability to establish rapport quickly with patients, families, and healthcare team members.
  • Excellent verbal and written communication skills.
  • Basic knowledge of clinical practices and patient-centered care principles.
  • Strong problem-solving and critical thinking abilities with sound judgment in knowing when to escalate concerns.
  • Organizational skills to manage multiple tasks with accuracy and efficiency.
  • Proficiency in Microsoft Office applications (Word, Excel, PowerPoint, Outlook) and collaboration platforms such as Teams or Webex.
  • Ability to work in a fast-paced environment while maintaining attention to detail and accuracy.
  • Compassion, empathy, and sensitivity when interacting with diverse patient populations.

Certification and Licenses:

  • Basic Life Support (BLS) Certification (Preferred, not required).

CONTACTS INSIDE AND OUTSIDE THE ORGANIZATION:

  • Frequent contact with Patient Coordination leadership and team members.
  • Regular interaction with patients and families via phone.
  • Intermittent contact with clinical staff for issue escalation.

WORK ENVIRONMENT AND PHYSICAL DEMANDS:

  • Professional office work setting.
  • Continuous oral and written communication.
  • Continuous computer use.
  • Occasional bending, kneeling, and light lifting up to 10 pounds.
  • Requires strong mental focus and attention to detail.

PRIMARY LOCATION:

  • Atlanta, GA (In Person role in Corporate Office)

SCHEDULE:

  • 25 hours a week for seasonal coverage (December & January)
  • Monday - Friday: 9am to 2pm EST (can accommodate afternoon hours if necessary which would be 12pm - 5pm)
