Position Summary

We are seeking an accurate, detail-oriented Medical Coder II to join our team. This role is responsible for reviewing medical records, identifying and validating diagnosis codes, and ensuring adherence to risk adjustment models and compliance standards. The Medical Coder II plays a key role in maintaining coding accuracy, documentation integrity, and supporting the organization’s mission of improving patient outcomes.


Essential Duties and Responsibilities

  • Review patient charts and medical documentation to ensure accuracy, completeness, and proper coding using ICD-10-CM and CPT guidelines.

  • Validate medical records, including history, exams, physician orders, consultation reports, and diagnostic summaries.

  • Identify chronic or new conditions and communicate findings to providers for validation.

  • Conduct ongoing analysis of medical charts to ensure coding compliance with federal and state guidelines.

  • Ensure adherence to all applicable laws, regulations, and internal coding policies.

  • Collaborate with team members to achieve department objectives and maintain strong interdepartmental relationships.

  • Meet daily coding productivity and accuracy standards.

  • Attend departmental meetings and training sessions as required.

  • Perform additional duties as assigned by the Sr. Risk Adjustment Manager or Lead Coder.

Qualifications


Qualifications and Requirements

  • Certification: CPC (Certified Professional Coder) – required; CRC (Certified Risk Adjustment Coder) – preferred.

  • Education: High school diploma or equivalent required; bachelor’s degree in a related field preferred.

  • Experience: Minimum of 3 years of medical coding experience, preferably in risk adjustment.

  • Proficiency in ICD-10-CM, CPT coding, medical terminology, anatomy, and physiology.

  • Knowledge of Medicare and federal/state reimbursement guidelines.

  • Strong computer skills, including EMR systems and Microsoft Office Suite.

  • Excellent attention to detail, communication, and organizational skills.

  • Ability to multitask and manage time efficiently in a fast-paced environment.


Working Conditions

  • Standard office environment; occasional local travel to care centers or continuing education seminars may be required.

  • Must be able to sit, stand, and use computer equipment for extended periods.

  • May occasionally lift up to 15 lbs.
