The Long Term Care Activities Supervisor is responsible for the management and oversight of the Activity Assistants, resident activity programs, both group and individual, resident required assessments, Minimum Data Set (MDS) documentation, care planning and other duties as they relate to the resident activity program. The quality of life for each resident will be the foundation of each activity program for each resident in the long term care setting. The Long Term Care Activities Supervisor is responsible for maintaining staffing levels, activity programs for all levels of resident function, developing employee talents, adopting new processes, technology and products to ensure successful innovation. Job Description
Essential Functions:
- Assess patients using information from observations and families as appropriate and develops a treatment plan individualized to the patient’s needs and interests.
- Educates patients and staff in recreational opportunities that address the special needs and behaviors of specific patient age groups.
- Supervises for appropriate use of activities supplies and equipment on a daily basis.
- Actively recruits and works with volunteers and is involved in promoting public relations within the community.
- Uses all pertinent data in planning patient’s care and records information in patient’s chart.
- Thoroughly documents assessments and resident issues in the medical record.
- Communicates findings to appropriate departments. Accurate documentation of care conferences is completed timely.
- Ensures new ideas are adopted and implemented to continually support the Healthcare mission while successfully accomplishing those goals the organization must attain for its strategy to succeed. Ensures employees share knowledge, understand decisions and the reasons why decisions are made and implemented.
- All other duties as assigned.
Additional Requirements
Required:
Education - High School Diploma/GED Equivalent in General Studies
Certification - Within 12-months of hire, have either:
A) Therapeutic Recreation Specialist or Activity Professional eligible for certification from National Certification Council of Activity Professionals (NCCAP);
or (B) Has 2 years of experience in a social or recreational program within the last 5 years, one of which was full-time in a therapeutic activities program;
or (C) Is a qualified occupational therapist or occupational therapy assistant;
or (D) Has completed a training course approved by the State.
Physical Requirements:Medium work - Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
