Who You’ll Join:

Join Our Team as an Ambulatory Care Pharmacist at Santiam Hospital and Clinics!
 

Santiam Hospital and Clinics is looking for a talented Ambulatory Care Pharmacist to join our team of medical professionals. The successful candidate will play a critical role evaluating medication appropriateness for our patients and provide expert recommendations, particularly for conditions not covered by Collaborative Drug Therapy Management (CDTM) agreements.

 

Salary range: $62.56 - $87.64 (DOE)

 

 

What You’ll Do:

  • Ethically and lawfully execute and complete Registered Pharmacist duties.
  • Through initiation and maintenance of applicable Collaborative Drug Therapy Management (CDTM) agreements, provide comprehensive medication management for (primarily) Medical Clinics patients and (secondarily) hospital inpatients who have diabetes, take anticoagulants, wish to quit smoking, or meet other conditions.
  • Monitor ongoing patient medication appropriateness and offer suitable recommendations for patients with conditions that are not covered by CDTM agreements.
  • Additional Professional Development
  • Maintain educational requirements for all required licensure and certification.
  • Attend professional conventions and in-services on current pharmacy topics.
  • Annually demonstrate competence in Department equipment operation.
  • Perform other duties assigned by Pharmacy Director.

Qualifications


What You’ll Need:

  • Minimum one year experience as a pharmacist in ambulatory care.
  • Aptitude in verbal and written communication, including ability to communicate with persons in all age groups.
  • Possession of a degree from an accredited pharmacy school.
  • Possession of current Pharmacist licensure by the Oregon Board of Pharmacy.
  • Possession of United States proof of citizenship or right to work in the United States.
  • Possession of a driver license or analogous identification.
  • Willingness to participate in Hospital orientation and educational in-service.
