POSITION SUMMARY

  • Responsible for the delivery of speech therapy services to patients in accordance with an established physician plan of care and in compliance with applicable licensure/certification requirements, Texas state practice act, and the agency's policies and procedures.

POSITION REQUIREMENTS

  • Minimum Education
    • Master's Degree in Speech Pathology from an accredited college and an Certificate of Clinical Competence in speech and language pathology.
  • Minimum Work Experience
    • Two or more years of clinical experience desired.
  • Required Licenses/Certifications
    • Current licensure in speech pathology as required by the state of Texas.
  • Required Skills, Knowledge, and Abilities
    • Specialized training in MBS procedures and evidence of competency in performing modified barium studies preferred. Effective verbal and written communication skills. Ability to meet minimum hiring standards.
  • Preferred Qualification
    • Specialized training in MBS procedures and evidence of competency in performing modified barium studies.

PHYSICAL REQUIREMENTS

  • Must be able to meet the physical demands which include standing, Walking, bending, lifting, and transferring; may involve lifting or positioning of patients. Must be able to utilize good judgment, demonstrate patience, and maintain a professional demeanor. Must be able to sustain concentration in a busy and stressful environment. Must work varied hours and schedules. Requires corrected vision and hearing to normal range. May be exposed to communicable diseases and bodily fluids. May be exposed to violent or disturbed individuals.

JOB SPECIFIC FUNCTIONS

  1. Demonstrates an understanding of and adherence to the HMHD Compliance Plan.
  2. Conduct reflects HMHD's values and a commitment to HMHD's Code of Conduct.
  3. Attends the required corporate integrity and compliance training and education programs.
  4. Demonstrates proficiency in understanding the materials presented during the corporate integrity and compliance training and education program.
  5. Complies with all HIPAA standards.
  6. Delivers direct speech therapy services to patients as ordered by a physician and in accordance with an established plan of care.
  7. Evaluates the patient within 72 hours.
  8. Plans and develops appropriate speech therapy care plan based on evaluation and as approved by the attending physician.
  9. Documents patient evaluation on appropriate forms in a timely manner.
  10. Develops a plan of care and consults with physicians and other members of the healthcare team on an ongoing basis for updates and communication.
  11. Documents short-term and long-term goals and includes discharge planning activities.
  12. Includes appropriate staff and family members in goal planning to provide continuity of care.
  13. Utilizes community resources and agency personnel by proper referral methods.
  14. Recommends communication devices and aids as indicated and according to patient capability.
  15. Administers speech therapy program utilizing specialized therapeutic techniques and/or equipment.
  16. Documents progress in meeting goals and/or documents reasons for failure to meet goals.
  17. Maintains and submits all required patient documentation relative to care delivered to the patient.
  18. Documents after each visit.
  19. Maintains appropriate documents required for billing.
  20. Confers with hospital staff and family members regarding methods of assisting patient to improve, correct and/or accept level of disability.
  21. Contacts physicians as necessary and appropriate concerning treatment and progress of patient.
  22. Demonstrates knowledge of and compliance with all hospital policies related to departmental operations.
  23. Complies with all local, state and federal regulations and JCAHO guidelines regarding speech-language therapy services and attends staff meetings as requested.
  24. Conducts staff in-services' as requested.
  25. Attends ongoing continuing education programs and in-services.
  26. Participates in performance improvement activities as requested.
