Community Health Worker - Bronson Womens Services - Downtown KalamazooThe Community Health Worker (CHW) will be responsible for increasing positive maternal-infant health outcomes for medically underserved populations. The CHW will coordinate efforts amongst staff and community groups to offer value-added resources and services during community events and interactions. The CHW will work to improve health outcomes of medically underserved population through empowerment, advocacy, and partnership collaboration. The CHW will nurture and establish trusting community-based relationships in an inclusive and culturally competent manner in order to help meet data driven screening and prevention needs.
QUALIFICATIONS - EDUCATION AND/OR EXPERIENCE
High school diploma or equivalent - must be 18 years of age or older (required).
Valid Michigan Driver’s License, reliable transportation and ability to travel independently to various locations (required).
Experience working with at-risk populations and their families (such as; low-income households, underinsured, and medically underserved) in a community-based setting for at least 2 years (required).
Candidate is from the community being served (required).
Associate or Bachelor’s Degree in health & human services field, social work, psychology, human biology, public health and community health or related field (preferred).
Successful completion of a formal Community Health Worker training program or willingness to obtain formal Community Health Worker training within 6months of employment (required).
OTHER SKILLS AND ABILITIES
Basic computer skills and related software application. Knowledge of basic medical terminology preferred or willingness to learn. Knowledge of community assets and resources. Understands the needs of medically underserved populations. Ability to plan, implement and evaluate community level events and initiatives. Skills in organizing resources and establishing relationships. Job produces some physical demands. Position will require travel and out of office community-based work up to 80 percent of the time. Typical of jobs that include regular walking, standing, stooping, bending, sitting and some lifting of lightweight objects. Work will require the operation of a personal computer provided by Bronson. Weekend hours may be expected.
Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time. Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects.

Collaborates with staff and community stakeholders to design and implement interventions that pursue a team-based approach to community engagement and service delivery.
Leans on lived experience and community expertise to inform staff of emerging needs for patient populations.
Has a proven track record of working in the community and providing community-based resource linkage.
Requires the ability to implement surveys, conduct interviews and lead focus groups.
High level of comfort in speaking with diverse populations, outside of your own, about their bodies, health behaviors, and the Social Drivers of Health (SDoH).
Possess organizational skills, is self-directed and enjoys problem solving.
Enjoys bringing people together and thrives in a team setting.
Excels working independently with direction.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Knows the importance of working collaboratively within the Bronson system and seeks to familiarize one’s self with the available in-house resources and practices.
Familiarity with community-based agencies and resources and makes appropriate referrals.
Helps to bridge conversations and remove barriers that prevent community members from accessing health and social services.
Understands the role of building healthy relationships as a conduit for creating and maintaining trust in healthcare.
Develops strong relationships and networks with communities that are in Bronson’s service area to improve health outcomes for community members.
Educate community members about the importance and availability of maternal-infant supports for those that are pregnant or postpartum.
Ability to set goals, assess progress and meet identified deliverables.
Ability to be innovative and think outside of the box.
Empathic and passionate about improving the health outcomes of medically underserved populations.
