Under the direct supervision of the supervising RN, the PACE Licensed Vocational Nurse (LVN) assists in assessing for and delivering clinical skilled services. This position is based in Home Care and collaborates with all PACE departments including the Clinic, Day Center, and supports PACE activities, including on the delivering assistance with Participant activities in the Center and Home environments as assigned.

Compensation: $31.09 – $32.64 an hour

Department: Home Care

Location: Modesto Center

Schedule: 8:30am – 5:30pm

Essential Duties and Responsibilities

  • Screen potential participants in the clinic, day center or home and assists the Supervising RN in assessing their environment, personal care needs, identify and deliver skilled needs.
  • Gather, compile and submit all information during Participant encounters.
  • Under supervision by the Supervising RN, assists in completing the assessment tool including diagnoses, medications, ADL's (Activities of Daily Living), IADL's (Instrumental Activities of Daily Living) in addition to a narrative summarizing potential participant's skilled needs.
  • Observe patients’ physical and mental wellbeing along with current medical status and/ or functional capabilities of each participant and report findings to MD or supervising nurse.
  • Track whether assessments have been completed.
  • Review medical records for each participant, on-going services and services received within past year using EMR, plan of care for the upcoming year using EMR, and IDT annual assessments to assist in ensuring continuing skilled needs are identified.
  • Perform basic wound care and check vital signs as directed by attending medical provider.
  • Administration of immunizations, injections, and medication as prescribed within scope.
  • Ensure documentation is complete; document medical history within the system, keep medical record current and upload as needed.
  • Supervise Patient Care Assistants in accordance with regulations.
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Drive to participant’s homes as needed.
  • Be flexible in schedule of hours worked.
  • Other projects and duties as assigned

Physical Demands

  • Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. 
  • Ability to exert maximum muscle force to lift, push, pull or carry patients.
  • Must be able to hear staff/clients on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
  • Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents
  • Must have high manual dexterity.
  • Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.

 

Work Environment

  • Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
  • Subject to unpleasant odors.
  • The noise level is usually quiet to moderate, but may at times be noisy and crowded.

Education/Experience Requirements

Minimum Qualifications

  • Valid CA Driver’s License, acceptable driving record, and vehicle insurance.
  • Must have reliable transportation.
  • Develop rapport, establish, and maintain effective relationships with internal and external contacts at all levels and from varying cultural and ethnic backgrounds.
  • Ability to work under stressful conditions.
  • Must have integrity, practice discretion and practice objective problem solving.
  • Data collection skills and knowledge of basic statistical principles.
  • Skilled in establishing and maintaining effective working relationships with participants, coworkers, medical staff, and the public.
  • Skilled in identifying and recommending problem resolution.
  • Knowledge of safety and infection control requirements for healthcare facilities.
  • Demonstrated experience in quality assurance and performance improvement activities and in the development and implementation of staff education with application of adult learning concepts.
  • Knowledge of nursing best practices, Medi-Cal, Medicare, and community engagement a plus, available community resources.
  • Proficient in Microsoft Office applications.
  • Veni Puncture Certified obtained with-in 1 year of hire.
  • Maintains professional affiliations and any required certifications.
  • Bilingual English/Spanish preferred.

 

 

Education/Experience

  • High school diploma or equivalent.
  • Graduate of an accredited school of professional nursing.
  • Current unencumbered Licensed Vocational Nurse (LVN) in the state of CA.
  • Current BLS CPR Card certified by American Heart Association or American Red Cross.
  • Minimum one (1) year direct patient care experience with Home or Community Health Provider experience.
  • Minimum one (1) year experience working with the frail or elderly population.
