Position Summary:
This position is responsible for individual service plan coordination of assigned mentally disabled clients, for the purpose of promoting their independence within the community, under appropriate clinical supervision.
Key Responsibilities:
- Case Management and Counseling:
- Provide individual, group, and family case management and/or counseling under clinical supervision.
- Ensure services meet productivity and quality requirements set by the Agency and within the scope of practice.
- Service Standards:
- Meet or exceed standards for services provided, including the required number of billable hours.
- Ensure assessments and documentation meet agency standards.
- Provide authorized services per Utilization Management, requesting additional services when needed.
- Provides only those services authorized by Utilization Management and provides services within the authorized frequency and duration of services. Requests additional services when initial authorization has been met.
- Individualized Service Plan and Discusses time criteria with specific goals and objectives that are measurable are developed with the consumer and signed at the time of the initiation of services and at time of each update.
- Assessment, Individualized Service Plan, Progress Notes and other documentation clearly identifies consumer’s input in services by having consumer’s and parent’s/guardian’s (if applicable) signatures when clinically appropriate.
- Completes progress notes at the end of each session and turns them in within 24 hours of the service.
- Notifies consumer and referral source (when applicable) prior to completion of the discharge summary and documents this notification on discharge within 30 days of last contact.
- Develops and maintains quality, accurate clinical charts for all clients served.
- Develops and maintains positive collaboration with other social service agencies serving clients. Maintains knowledge and familiarity with referral sources within the community. Provides consultation when needed.
- Has no unexcused absences from any unit, department, and mandatory Agency meetings and training.
- Maintains license or registration appropriate to profession and appropriate to job requirement, including appropriate amount of continuing education at all times of employment.
- Maintains agency-required annual training including client rights and grievances, safety training, infection control, and others as required by OHMHAS.
- Remains free from citations by other providers or by consumers for unethical practice and meets the standard for their own profession.
- Adheres to all organizational and departmental policies, including compliance with all behavioral and ethical expectations.
- Participate in at least 6 hours of supervision for every 3-month period or meet the requirements of supervision required by license/registration (whichever is greater).
- Other clinical duties as assigned.
Qualifications
Minimum Qualifications:
- Education: Associate degree in human/social service field, or High School Diploma with equivalent field experience
- Experience: 3 years working with SMD population
- Certification: First Aid; CPR; De-escalation.
- Other: Valid Ohio driver’s license; proof of automobile liability insurance; meets OMHAS licensure and certification requirements; competencies for serving the SMD population.
Knowledge, Skills, & Abilities:
- Knowledge of: Safety rules and procedures, Agency policies and procedures, county area and resources, daily living tasks and skills, documentation requirements.
- Skills: Driving, operating basic household appliances and tools, directing, and demonstrating activities of daily living, i.e., Cleaning, laundry, grocery shopping.
- Abilities: Lifting up to 25 pounds, writing legibly or use of computer, ability to see, hear, read, and communicate information clearly, ability to recognize unusual or threatening conditions related to persons with disabilities.
Skills:
- Capacity to perform duties effectively under potential emotional stress and conflict situations.
- Proficiency in utilizing basic computer skills and office equipment for documentation and communication purposes.
- Practice of a client-focused approach, demonstrating active listening skills, reading comprehension, critical thinking, and leadership abilities.
Abilities:
- Willingness and physical capability to engage in activities essential to meet the fundamental needs of individuals served. This includes occasional assistance in relocating individuals to new living situations, aiding with shopping, and instructing daily living skills, as necessary.
