DIRECTOR OF CLINICAL CARE JOB DESCRIPTION
Job Summary: The RN Clinical Manager is responsible for providing direct care and case management for a team of staff to care for patients in accordance with agency standards and policies, and state and federal regulations. The RN Clinical Manager performs skilled nursing functions in the home, community, and on-campus setting to serve the nursing needs of patients and families/caregivers for hospice care.
Hours of Work: Monday through Friday; 8:00 a.m. to 4:30 p.m. Must be flexible for patient needs. Rotating on-call duties, with evening and weekend visit expectations.
Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity.)
- Education:
- Graduate of an accredited school of nursing.
- Current Minnesota registration as a Registered Nurse. Must maintain licensure.
- Current CPR Certification or ability to immediately obtain certification
- Experience:
- Strong proficiency in nursing skills.
- Hospice experience preferred.
- Previous management experience preferred.
- Basic Computer skills.
- Personal:
- Highly developed communication skills (oral and written).
- Proven time management skills.
- Understanding of basic quality principles.
- Adaptable to various work settings.
- Self-motivated.
- Attention to detail.
- Problem solving skills.
- Ability to direct and supervise.
- Ability to ensure confidentiality.

- Provide a pleasant attitude and compassionate disposition towards staff, patients, and families.
- Own vehicle, current valid driver’s license, and proof of current automobile insurance.
- In accordance with CMS Vaccine Mandate, must provide proof of Covid-19 Vaccine or submit documentation for exemption
- Job Knowledge:
- Knowledge of nursing policies and procedures.
- Knowledge of infection control practices.
- Knowledge of State Federal regulatory requirements.
- Knowledge of the Nurse Practice Act.
- Ability to assess patient’s condition with knowledge to provide necessary action.
- Supervisory knowledge and abilities.
- Philosophical understanding of quality of life and quality process management.
