Qualifications


Job Summary:

The position serves as an intake assessment nurse and will support in other nursing functions as needed.

 

RN Duties/Responsibilities:

  • Performs assessments to determine medical eligibility for program.
  • Educates qualified leads as to what can be expected within the PACE program.
  • Coordinates intake activities with other intake team members including medical and financial eligibility and enrollment activities.
  • Reports qualified leads to the team for safety in the community review.
  • Provides back-up nursing assistance as needed and directed by DON.
  • Participates in Interdisciplinary team meeting, safety in the community the community review, care planning as needed, and other meetings and committees as directed by the DON and/or Program Director.
  • Utilizes the Health Information Exchange to gather information about candidates to complete intake assessments.
  • Assists with developing annually departmental policies and procedures.
  • Tracks and reports mileage and other expenses using the HR software in a timely manner.
  • Other duties as assigned.

RN Education, Licenses, Certifications and Experience:

  • Current Registered Nurse License
  • Current CPR Certification
  • UCAT certified preferred

LPN Duties/Responsibilities:

  • Provide nursing services as indicated in Participants care plan or by physician/provider orders
  • Provide care as needed in the center or the participants’ home.
  • Identify areas of concern or needed improvement to maximize participant satisfaction and/or positive health outcomes, promotes those ideas to the RN Care Coordinator or IDT
  • Provides back-up to other nurses, home care and clinical staff as needed.
  • Administers medication and provides medication education to participants and their families.
  • Provides participants and families education as needed
  • Participates in training opportunities and in-services
  • Accurate and timely documentation.
  • Any and all other duties and responsibilities as assigned.
  • Participate in on call rotation

LPN Education, Licenses, Certifications and Experience

  • Current Oklahoma LPN License without restrictions
  • Either one year working with the frail and elderly population or approved training.
  • Current CPR certification
