Description

Sangre de Cristo Community Care is looking for a Part Time Time Bachelor’s Level (Master’s preferred) Social Worker to join our dynamic team in our La Junta, CO office. Sangre de Cristo Community Care is the Southern Colorado leader in enhancing the quality of life and maintaining the integrity of individuals and their families in need of supportive or end-of-life services by offering symptom management, support, comfort, and compassion.

Responsibilities:

  • Provides psychosocial support to patients and families by provided skilled social work invention
  • Provides resources, referrals, education, supportive counseling, discharge planning and assistance with final arrangements
  • Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
  • Plans, coordinates, and manages the volunteer program
  • Supervisors and trains new volunteers
  • Identifies needs for volunteers and assigning of volunteers to patients and families

Requirements

  • A Bachelor’s Degree in social work, counseling, or related field from an accredited university (Master’s preferred)
  • Must have a valid non-restricted driver’s license
  • Bilingual in Spanish preferred
  • Flexibility and availability to work a weekend schedule if needed.
  • At least 1 year of healthcare experience preferred.
  • Previous volunteer coordination preferred
