Are you passionate about supporting those who have served our country? NCSS is seeking a dedicated Care Coordinator to join our team and provide integrated, person-centered support to military members, veterans, and their families.
In this role, you’ll play a key part in ensuring individuals receive timely, coordinated care that reflects their goals and needs. You’ll collaborate across systems, working closely with healthcare providers, community agencies, and the Department of Veterans Affairs—to help clients access the right level of care, navigate benefits, and transition smoothly between services.
The ideal candidate thrives in a fast-paced, team-oriented environment and values quality, compassion, and collaboration. You’ll support ongoing case management efforts, assist with administrative and documentation processes, participate in quality improvement initiatives, and maintain close communication with clients and partners to promote engagement and wellness.
Your work will focus on empowering veterans and service members by addressing unique needs such as trauma recovery, reintegration challenges, housing stability, and behavioral health coordination. This position requires flexibility, strong communication skills, and a commitment to trauma-informed, culturally competent care.

Qualifications
Bachelor’s Degree preferred; or equivalent combination of related education, training and experience. Experience working with or within the VA, Department of Defense, or veteran-serving nonprofits is highly valued.
