KEY JOB RESPONSIBILITIES: Tasked with the responsibility of taking care of terminally ill patients at home. Operates in a systematic manner that takes care of the whole gamut of healthcare for terminally ill, death-bound patients. Provides care that is well-tailored to the cultural, religious, and social needs of a dying patients.
Additionally, the Nurse Practitioner:
- Regularly assesses and reassesses the nursing needs of the Hospice patient.
- Performs Face to Faces timely.
- Provides dietary counseling.
- Provides Hospice nursing services, treatments, and preventive procedures.
- Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
- Observes signs and symptoms and report to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
- Teaches supervises and counsels the Hospice patient and family members about providing care for the patient.
- Supervises and trains other nursing service personnel.
- Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
- Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.
- Attends team conferences.
- Maintains records as required by Hospice.
- Follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
- Always communicates to the supervisor if unable to meet a patient’s need or perform a procedure.
- Participates in the on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
- Maintain skills and knowledge.
- Works with interdisciplinary group concept of patient care.
- Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF, or MR.
- Organizes work schedule and utilizes time management to be able to attend all required meetings.
- Complies with agency infection control policies and protocols.
- Assist with orientation, teaching, and training as requested.
- The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patient’s home (including private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
- Other duties as assigned by Director.
Qualifications
- Help optimize the capital structure and support strategic initiatives.
- One must be able to hear adequately on the telephone with no more than an amplifier and be able to communicate both verbally and in writing in English.
- Maintains the agency’s mission, philosophy, and core values.
- Adheres to dress code, appearance is neat and clean, wears identification while on duty.
- Reports to work on time and/or as scheduled.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
- Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
- Always maintains patient confidentiality, including all HIPAA regulations.
Regulatory Requirements
- Licensed or registered in which he/she currently practices.
- Must pass a criminal background check & MVR check.
- Completed health statement acknowledging ability to perform the duties of the position.
- Valid state driver’s license.
- Proof of current automobile insurance.
- Current CPR card; Online CPR certification accepted; Hands-on CPR certification preferred.
- TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
Industry experience
- Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.
- Minimum 2 years of experience as a registered nurse, preferred.
- Active patient contact within past three years, preferred.
EDUCATION:
- Graduate from an accredited school of nursing.
- Currently licensed as a Registered Nurse and Nurse Practitioner to practice in the state.
