POSITION SUMMARY: The Clinical Team Lead is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Team Lead is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. 

 

ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING: 

  • Develops and initiates the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, family, and caregivers.  

  • Initiates proper preventative and rehabilitative nursing procedures and provides services as ordered by the physician as indicated in the plan of care. 

  • Ensures a Clinical Manager/Supervisor is available during all operating hours.  

  • Assures the development, implementation, and updates of the individualized plan of care, including communication with all physicians involved in the plan of care and integration of orders, including medication orders.  

  • Receives case referrals and accepts those that the agency can appropriately serve.  

  • Reviews available patient information related to case, including home visits, to determine home health care needsAssigns appropriate home care personnel to case as needed.   

  • Attends conferences with attending physician regarding any questions about an individual's eligibility for services. 

  • Reviews and evaluates each case by assessing the services provided by clinicians, case conferences, performing record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians, as appropriate. 

  • Reviews patient's medical diagnosis, procedures, medications, and clinical course.  

  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care. 

  • Attends, leads, and documents case conference meetings with agency personnel to facilitate coordination of care. 

  • Participates in quarterly record reviews and communicates findings and recommendations to Director of Nursing and appropriate agency personnel. 

  • Assists in the screening and interviewing process of new agency personnel and makes recommendations for employment of individualsAssists in the orientation of new agency personnel. 

  • Assists Director of Nursing in the planning, implementation, and evaluation of in-service and continuing education programs. 

  • Complies with accepted professional standards and principles. 

  • Participates in public relation and community activities that promote the agency's role as an effective member of the health care delivery system. 

  • Complies with accepted professional standards and principles.

  • Participates actively in quality assessment performance improvement program. 

  • Performs other duties and activities as delegated by the Director of Nursing and/or Branch Manager.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION: 

  • Must hold current licensure as a Registered Nurse, Physician, Physical Therapist, Occupational Therapist, Speech-Language Pathologist, or Social Worker, in state of practice. 

  • Two (2) years of supervisory and/or management experience in home health or related healthcare setting preferred.  

  • Current CPR certification required. 

  • Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation. 

 

PREFERRED KNOWLEDGE AND SKILLS:   

  • Excellent time management, observation, and problem-solving skills. 

  • Must have outstanding verbal and written communication skills. 

  • Must be self-motivated and able to work independently while being collaborative and able to work as part of an interdisciplinary team. 

 

SUPERVISORY RESPONSIBILITIES: 

  • May have supervisory responsibilities of employees doing similar or related work in the same department.
