General Position Description:
The Admission Nurse is a Registered Nurse responsible for promoting the delivery of coordinated, comprehensive care to patients through the activities of assessment, planning, documentation, and direct nursing care within the policies and procedures of the Agency. The Admission Nurse is responsible for accurately completing the OASIS assessment, administering skilled nursing care to patients, and determining what additional disciplines and/or programs are required for the patient's care. The Admission Nurse makes the evaluation and assessment visits for an assigned geographic region, which may include the entire agency location, using a point-of-care electronic documentation platform. The Admission Nurse will be required to educate staff and deliver training programs within an assigned geographic region.
Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.
Essential Functions:
- Make initial evaluation visits as applicable and consult the attending physician to establish the care plan. Responsible for timely communication of the care plan to the RN case manager, other members of the interdisciplinary team, and the clinical manager. Complete SOC OASIS and develop an appropriate plan of care based on physician orders using an electronic point of care documentation system. Submits documentation in a timely and complete manner according to agency policy
- They may be required to provide clinicians with training and follow-up assistance on plan of care development, appropriate OASIS documentation, and utilization of clinical specialty programs. They may also be required to do Recertification visits and resume-of-care visits/oasis assessments.
- Required to participate in the On-Call rotation of an assigned geographic region.
- Participates in continuing education r/t OASIS completion and review, ICD-10 coding and industry standards and trends as directed by the Administrator.
- A minimum of 6 hours of ICD-10 coding-related education is required annually. (Agency reimbursed)
- Productively, the expectation is 3-4 oasis visits per day, depending on availability and geography.
- May be required to perform other skilled nursing visits as needed.
- Meet mandatory continuing education requirements of the agency/licensing board.

Requirements
Supervision:
LVN/LPNs and CNAs as necessary
Experience:
- A minimum of three (3) years of professional experience with at least one (1) year of home care experience is required.
- One year of experience with an electronic medical record
Skills:
- Excellent knowledge of Federal and State regulations about home health.
- Excellent knowledge of home care standards and practices.
- Ability to multitask with strong problem-solving skills
- Excellent written and verbal skills
- Strong customer service skills
- Ability to travel as required to perform job functions
Education:
- Completion of an accredited Registered Nursing Program.
- Preferred, BSN.
Licensure/Certification:
- Current driver’s license in good standing. The employee is responsible for renewing their driver’s license before it expires to continue employment.
- Reliable transportation and auto liability insurance.
- Current State professional license as an RN in any/all applicable states, BSN preferred
- OASIS and/or ICD-10 coding certification preferred.
- Current CPR.
- If not certified, one must be willing to become OASIS certified (at the company’s expense) within 6 months of being hired in the position.
Ongoing employment as an RN always requires maintenance of a valid RN license in the state where the nurse practices and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.

Environmental/Working Conditions:
- Works in the patient’s home in various conditions.
- Possible exposure to bloodborne pathogens, bodily fluids, and infectious diseases.
