Job Description
The Child and Adolescent Clinical Resource Nurse and Outpatient Case Manager serves four primary functions on the Child and Adolescent Program (CAP) Campus: ·
- Develop, coordinate and train clinical education of staff across the continuum of child and adolescent services. This may include facilitating interdisciplinary training on nursing practice/concepts as part of integration into the milieu and groups.
- · Coordination of outpatient appropriateness for admission through initial screening, assessment and patient intake, throughput, and transitions from referral to discharge as well as ensures the application of evidence-based nursing care for all aspects of the Intensive Outpatient Program (IOS) per the regulatory requirement of Wisconsin Chapter 40.
- Facilitation of continuity of care for CAP outpatients. This includes facilitation of transition from inpatient to outpatient status and facilitation of ongoing outpatient services with third-party providers; provides appropriate outpatient nursing care, and acts as the conduit between primary care and outpatient/community based health care and mental health providers while the patient is a participant in the IOS program.
- ·Assists with utilization management to ensure optimal clinical and financial outcomes of the outpatient program. This includes working with third party payers to justify initial and ongoing care through documentation and outcome data.
This position reports to the Manager of Outpatient and Ancillary Services but works closely with the Inpatient RN Manager to ensure clinical education development and training meets the needs of all staff on campus.
The two Clinical Resource Nurse and Outpatient Case Coordinator positions are designed to work complimentary shifts in order to cover the full hours of operation for the IOS program and for some availability in the evenings and early mornings for education opportunities for off-shifts.

- BSN - will consider other nursing degrees on an individual basis.
- 1-2 years of nursing experience.
- Electronic medical record experience
- Experience working with children and adolescents.
- Valid driver’s license when driving any vehicle for work-related reasons.
- Currently licensed as Registered Nurse and able to practice in the State of Wisconsin.
- Reads, writes and is fluent in the English language.
- Proficient computer knowledge and skills to include: word processing, spreadsheets, power point creation, email, web browsers and electronic medical record.
- Ability to adapt and flex to dynamic clinical environments and to the needs of learners.
- Ability to work independently and in collaboration with other key stakeholders.
- Consistently meets deadlines and department initiatives.
- Excellent planning and organization skills.
- Strong interpersonal and communication skills.
