Title: Aftercare Coordinator 

Location: New York 

Pay Range: $30.21 - $32.96 per hour (effectively $55,000 - $60,000 per year) 

FLSA Classification: Full-time (35 hours per week), Non-exempt 

 

Role Summary: The Aftercare Coordinator is responsible for providing aftercare services to former clients for up to one (1) year after their transition from shelter to independent living, ensuring they maintain stable housing and avoid returning to shelter. This role involves conducting home visits, connecting clients to offsite services and community-based organizations, and fostering cross-training to understand and support the functions of the Social Services Department. 

 

Primary Job Responsibilities/Duties: 

The Aftercare Coordinator is responsible for, but not limited to: 

  • Collaborate with shelter staff and the housing team to ensure successful housing placement.
  • Assist with client intake, service planning, advocacy, and referral services in the absence of a case manager.
  • Maintain accurate records and counseling session logs in the DHS CARES system.
  • Work closely with the site leadership and Department of Homeless Services to support clients in achieving permanent housing goals after transitioning from shelter.
  • Make appointments with landlords and other housing agents to ensure clients have housing leads in the event relocation from current housing is required.
  • Identify barriers to clients maintaining permanent housing and develop linkages to address these challenges.
  • Conduct monthly housing-related activities and workshops.
  • Perform aftercare visits, including monthly and quarterly follow-ups after clients move into permanent housing.
  • Connect clients with community-based social service providers to meet long-term social support and service needs.
  • Provide mediation and advocacy with existing or newly identified landlords or property management to address repairs or rental arrears.
  • Attend staff meetings and serve on committees as required.
  • Conduct monthly follow-up for up to six months after clients exit the program.
  • Perform general clerical duties.
  • Complete other duties as assigned.

 

Physical Requirements: 

  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to sit for extended periods and perform repetitive tasks.
  • Must be able to lift and carry up to 25 pounds.
  • Must be able to travel to multiple NYC sites as needed.
  • Must be able to access and navigate each department at the organization’s facilities.
  • Ability to climb stairs.  

 

Work Environment / Schedule Requirements: 

  • Office setting with regular exposure to computer screens and moderate noise levels.
  • May be required to work shifts, including evenings or night tours and/or Saturdays, Sundays and holidays with holiday pay differential. 
  • Transitional Housing Program staff are required to continue their shift until relieved by another staff member or authorized to leave by a supervisor.

 

Qualifications:  

  • High School Diploma / GED with two (2) or more years in case management. 
  • Experience working with diverse populations, including individuals experiencing homelessness. 
  • Proficiency in Microsoft Office Suite.
  • Strong engagement, interpersonal, and communication skills. 
  • Ability to facilitate group workshops.
  • Excellent organizational and problem-solving skills.
  • Knowledge of housing subsidies and grants available in NYC. 
  • Knowledge of case management software (e.g., CARES, HOME) preferred.
  • Ability to travel throughout the five boroughs of NYC. 
  • Must be able to read, speak, write, and understand English for administrative purposes.
  • Must pass drug screening to be appointed.
  • This position may be subject to a series of investigations before and after appointment.
