Part time Case Manager
Pay: $23.00 per hour

Through empowerment, advocacy, and community collaboration, we help our residents regain stability and build a path toward self-sufficiency.
Case Manager:
The Case Manager uses counseling and coaching to coordinate and ensure continuity of program services needed to promote client success. These services include assessment of urgent needs, development of case management plan, coordinating access to services, making referrals, monitoring client engagement, and advocating to help clients access resources.
Duties and Responsibilities:

Assist Sr Case Manager with client assessment in application process from pre-screening to intake ensuring efficiency and maximizing shelter capacity

Assess strengths and barriers of families and individuals including areas such as employment, housing history, behavioral health, criminal history, and substance abuse.

Assist families and individuals to establish housing and other goals, identify steps toward goal achievement and measures of evaluating progress (case management plan).

Make appropriate referrals for housing and rental assistance based on assessment and client input, advocating for client when needed.

Provide counseling, coaching, and community resource referrals for clients.

Maintain weekly client sessions to review/reinforce plan, provide encouragement, evaluate progress, and assist with goal modification when needed.

Apply active listening skills in client interactions.

Demonstrate awareness of escalating behaviors and apply appropriate de-escalation strategies.

Document case management services precisely, accurately, and in a timely manner to client files and program databases.

Coordinate with outside service providers to meet client needs and avoid duplication of services.

Assist with client educational pursuits and in-house life skills classes/workshops.

Assist alumni clients by providing available resources and increase engagement

Maintain appropriate client boundaries, client confidentiality, and HIPAA protocols.

Adhere to standards set forth in the Employee Code of Conduct regarding ethical behavior, confidentiality, and conflicts of interest.

Bilingual (Spanish) preferred.
Experience/Education:
-
Bachelor�s degree in a relevant field or equivalent experience specifically in social services, counseling, psychology, human services, or child development.

Minimum of three years� experience in case management.

Knowledge of and experience with housing laws and available resources.

Proficiency in the use of computers for word processing, basic financial management, data entry, email, and internet use.

Experience with HMIS a plus.

Familiarity with mandated reporting laws for protected populations (children and elderly).

Complete mandated reporter training within 90 days of hire date and register with Child Protective Services as mandated reporter.

CPR-First Aid certification (adult/child/infant).

Valid California Driver�s License and own transportation.
