Definition of Position:
The Outreach Specialist serves as a strategic communication liaison between the organization and external partners, including community health clinics, healthcare providers, and other stakeholder organizations. This position is responsible for ensuring effective coordination of service lines, facilitating the introduction of new providers and clinical services, and supporting collaborative relationships that enhance operational efficiency and community impact. Acting as both a connector and communicator, the Outreach Specialist plays a key role in aligning organizational processes with partner expectations, promoting new initiatives, and fostering strong, sustainable partnerships across the healthcare landscape.
Responsibilities:- Act as the main point of contact for external health clinics, providers, and community organizations regarding services, referrals, and partnerships.
- Communicate the launch of new service lines and clinical offerings clearly and consistently to partner organizations and providers.
- Coordinate the introduction and integration of new providers, ensuring smooth transitions and effective communication across teams.
- Streamline inter-organizational communication and workflows to ensure efficient service delivery and collaboration.
- Cultivate and maintain strategic relationships with healthcare and community-based organizations to enhance collaboration and service reach.
- Represent the organization in outreach efforts, meetings, and collaborative initiatives, sharing updates and gathering feedback.
- Support continuous improvement by identifying challenges in communication or coordination and recommending solutions.
- Work cross-functionally with internal teams (clinical, operations, communications) to ensure messaging and processes are aligned.
- The employee supports the hospital mission, vision, values, policies, and procedures.
- Participates in required education for DNV programs as applicable to position (reference program education curriculum).
- Performs other related duties as assigned.
Education
Required: Bachelor’s degree in nursing, Public Health, Healthcare Administration, or a related field.
Preferred: Masters preferred.
Licenses/ certifications/ registrations
Required: Registered Nurse license

Experience
Required: 3-5 years of experience in outreach, provider relations, or healthcare engagement.

Other:
Strong understanding of healthcare systems, service coordination, and referral networks. Exceptional communication and interpersonal skills. Ability to manage multiple partnerships and projects with attention to detail. Self-starter with a collaborative mindset. Experience working with diverse populations. Knowledge of local or regional provider networks and referral systems. Multilingual or bilingual proficiency is a plus.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. Occasional walking, standing, bending, or lifting light office items (up to 25 pounds). Visual acuity to view computer screens and read detailed financial documents. Ability to communicate effectively in person, by phone, and electronically. May be required to move throughout the healthcare facility, including administrative and clinical areas.
Working Conditions:
This position operates in a professional office setting within a healthcare facility. The role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The office environment is climate-controlled, well-lit, and ergonomically designed for extended computer work. While this role is primarily office-based and does not involve direct patient care, the employee may occasionally be present in clinical areas or interact with staff working in clinical environments. As such, there is a potential for exposure to infectious diseases. The organization provides appropriate training and personal protective equipment (PPE) as needed to ensure safety.
