Three Rivers Hospice, a division of Quality Life Services is seeking a Full-Time Social Worker in the Chicora, Sarver, Worthington, and surrounding areas who wants to make a difference in the lives of older adults. We are offering a $3,000 sign on bonus. Essential Functions, include but not limited to: 

• Assess the psychosocial status of patients / families / caregivers related to the patient’s terminal illness and environment
• Provide an assessment in the patient’s identified residence and assistance with this is not safe and another plan is required
• Carry out social evaluations and plan interventions based on evaluation findings
• Counsel patient / family / caregivers as needed in relationship to stress and other identified coping difficulties
• Maintain clinical record on all patients referred to social work
• Oversees and instructs medical social worker interns per specific college requirements
• Maintain collaborative relationships with organization personnel to support patient care

The ideal candidate has:

• Master's Degree in Social Work (MSW)
• Provides services for all hospice patients
• Can create professional relationships with current hospice patients and their families
