Hours: 35- FT M-F Flexible
Rate of Pay: $20.00
- Work with individuals who are receiving care management services.
- Work collaboratively with the care managers who are providing services to enrolled members.
- Work with families and children who are in need of care management services.
- Assist children in applying and receiving “Family of One” Medicaid, which would otherwise be denied due to their parents' income or resources.
- Connect the child (and family) to community resources and additional services.
- Knowledge of behavioral health systems, other health systems, and community resources.
- Strong communication and active listening skills.
- Ability to build trusting relationships with families, children, and adult individuals.
- Flexibility, adaptability, and the ability to work both independently and as part of a team.
- Strong problem-solving and advocacy skills to help individuals/families navigate complex systems.
- Cultural sensitivity and respect for diversity.
- Dependability, professionalism, and ability to maintain healthy boundaries.
- High school diploma or equivalent required.
- Certification as a Peer Specialist / Peer Advocate (or eligibility/willingness to obtain certification within a required timeframe).
- Lived experience with mental health and/or substance use recovery, and willingness to share that experience in a professional, supportive manner.
- Bilingual preferred.
