The Nurse Practitioner is a highly skilled professional member of the Interdisciplinary Team with the primary responsibility of providing Face-to-Face visits in alignment with the Hospice Medicare Benefit regulatory requirements and end of life care to hospice patients and their families within their scope of practice. The HCIB Nurse Practitioner has a key role to provide critical information to the Hospice Medical Director to aid him/her in determining patient eligibility for hospice services.

 

· Participate in the medical evaluation of patients who are residing in the hospital, home, ALF/ILF, and SNF.

· Offer advice and information to staff and referring physicians on medical interventions consistent with the hospice philosophy and plan of care.

· Consult with hospice staff as requested, so pain and symptom management for patients is effectively and expeditiously accomplished.

· Provide critical information through Face-to-Face encounters to the Hospice Medical Director to aid in determination of patient eligibility for hospice services in accordance with hospice regulations.

· Responsible for making contact with assigned patient and/or family within twenty-four hours to arrange a meeting to provide information to the prospective patient and/or family. An assessment of the patient’s eligibility for hospice services with be completed and communicated to the Hospice Medical Director.

· Based on patient need, consultation and evaluation of the patient will be completed.

· Responsible for documenting interactions between hospice staff, patients and families.

· Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care. Record pain and symptom management changes and/or outcomes as appropriate.

· Point of Service documentation is an expectation of all clinical personnel for quality care, accuracy, and communication standards. The clinician is expected to appropriately document at Point of Service, have timely synchronization of patient data & effectively communicate with the Patient Care Team.

· Present education and information to members of the IDT and/or medical community as needed.

· Participate in the resolution of interpersonal conflict and issues of clinical and ethical concern as needed.

· Participate in research activities or special projects as assigned and needed.

· Face-to-Face visits as assigned
