Essential Functions:
- Ability to case manage a diverse population of HIV-infected individuals
- Ability to respectfully interact with clients and work cooperatively as part of a team
- Ability to respond to emergencies
Duties and Responsibilities:
- To provide on-site Ryan White Medical Case Management services to an assigned caseload.
- Collaborate with clinic staff to ensure quality care and identify appropriate services.
- Establish and maintain a professional therapeutic rapport with clients.
- Conduct intake assessments and reassessments of clients.
- Develop a care plan and monitor client’s progress towards meeting the plan goals.
- Develop resources and make referrals appropriate resources.
- Follow up with client’s service providers regarding referrals.
- Respond to issues effectively and in a timely manner.
- Document activities regularly in progress notes.
- Protect the oral, written and electronic confidentiality of the client and the client’s records according to professional ethics, law, and HIPAA rules.
- Participate in case conferences with health care and other providers to sustain or improve client’s quality of life.
- Meet annual training requirements established by Ryan White funders and Zufall Health Center.
- Attend and participate in appropriate staff meetings and willing to travel between Zufall sites for clinical days.
- Report a client’s health and/or behavioral changes to the supervisor.
- Reports a client’s threats to harm self or others to the supervisor.
- Other duties as required and necessary.
Requirements
- Bachelor’s degree in public health, social work, health services, or related field required, or a combination of education and experience in lieu of a bachelor’s degree.
- Expertise and experience delivering care and case management in a clinical setting, preferable specific HIV case management experience or relevant healthcare/clinical experience in a clinical setting.
- Familiarity with HIV/AIDS, substance use, and mental health.
- Ability to work cooperatively as a part of a team in a clinical and non-clinical setting.
- Bilingual (Spanish)/bicultural preferred.
- Excellent oral and written communication, organizational, and computer skills.
