About the Role:
Are you passionate about helping patients navigate the healthcare system and improve their well-being? Join our team as a Clinic Patient Navigator! In this vital role, you'll support patients by bridging gaps between them, their families, providers, and the healthcare system. You’ll perform a mix of care coordination, clinical support, and health education—both within the clinic and out in the community.
This position is ideal for someone who is highly organized, compassionate, and committed to improving patient outcomes—especially for individuals with chronic conditions like hypertension, diabetes, and cardiac disease.
What You’ll Do
Patient Support & Care Coordination
- Coordinate new patient intake appointments, care plans, and medication authorizations/appeals.
- Conduct follow-ups (TCM calls) and schedule referrals to specialty providers.
- Reach out to patients for preventive and chronic care services.
- Remind patients of upcoming appointments and assist with visit coordination.
- Act as a liaison between patients, providers, and community services.
- Manage controlled substance background checks and home health billing forms.
Clinical & Administrative Support
- Room patients, record vital signs, update medical histories, and assist with exams.
- Perform EHR documentation, scribe during patient visits, and ensure accurate recording of labs and results.
- Administer medications, draw blood, and dress wounds.
- Call patients with test results or medication updates.
- Handle clerical duties: answering phones, scheduling, filing, supply ordering, and billing support.
Chart Extraction & Data Entry
- Extract historical health records and enter them into Athena Health.
- Input structured data from QHN and update health histories/problem lists.
What We’re Looking For
Education & Experience
- High school diploma or equivalent plus 5 years in a provider clinic setting
OR
Bachelor’s degree with progressive clinical or administrative experience. - Must be licensed/certified/registered as a CNA, MA, or LPN in Colorado within 18 months of hire.
Skills & Requirements
- Excellent communication, problem-solving, and customer service skills.
- Able to work independently while collaborating with a team.
- Strong computer skills (Microsoft Word, Excel, Outlook; EHR systems).
- Familiarity with medical terminology and documentation standards.
- BLS certification required.
- Fluency in English required; Spanish fluency preferred.
