Position Summary
The Perinatal Case Manager provides patient guidance and support, focused on social determinants of health and addressing barriers to care, and serves as an advocate for PrimeCare’s patients. In addition, the Perinatal Case Manager is responsible for educating and empowering patients through their maternal health journey, beginning at conception through their first six months post-partum. They will work one-on-one with patients to complete evidence-based activities that will lead to optimal outcomes for the parent(s) and child. This position will provide knowledge and expertise to pregnant and postpartum patients as well as other clinic patients to ensure that patients meet their health and wellness goals.
Duties and Responsibilities
- Serve as integral member of the care team: review daily schedules and patient charts, identify gaps in care and areas of need, schedule appointments, communicate needs and care plan with care team.
- Facilitate communication among patients, providers, nurses, medical assistants and outside resources, related to the care of patients.
- Promote the health of pregnant and postpartum patients.
- Conduct varying health risk assessments for prenatal patients, postpartum patients, and the general population and connect them to the appropriate community resources (insurance services, transportation, childcare, employment, legal, etc.) and provide consistent follow up to ensure services are received.
- Perform population-based outreach to close gaps in care and ensure timely patient follow-up including, but not limited to, vaccine schedules, cervical, colorectal, and breast cancer, positive pregnancy tests, well child visits, and postpartum visits.
- Manage PrimeCare’s blood pressure monitor distribution program and nebulizer distribution and provide education on use of equipment and general chronic condition management.
- Teach and counsel patients on different health topics such as hypertension and healthy pregnancy.
- Participate in specialized programs such as quality initiatives and workgroups, grant-funded programming, education programs, IBCCP coordination, etc.
- Facilitate Prescription Assistance Program applications, following up through completion to ensure patients have access to medications to manage their chronic conditions and contraception.
- Manage full process for Home Health/Homemaker services and Early Intervention Referrals starting with authorization, communication, and coordination with the specialist/external facility, and conducting provider and patient education throughout the process.
- Facilitate uninsured Shingrix application process, coordinating care between supplier and patient and the clinical site to ensure qualifying patients access vaccines.
- Manage all Durable Medical Equipment (DME) orders, collaborating with external DME companies, completing authorization, and ensuring patients receive required DME.
- Represent PrimeCare at community events, educating the community on our services and promoting our organization and programs.
Required Knowledge, Experience, or Licensure/Registration
- At least two (2) years of experience, preferred in social services. Background in a community health setting is highly preferred.
- Bachelor’s degree in related field preferred.
- Certificate in Maternal Health Coaching or related field, or plan to obtain certificate with 6 months of hire.
- Ability to communicate with licensed clinical staff including nurses, social workers, and providers.
- Excellent communication, listening and interpersonal skills; exhibits a sense of compassion and inspiration.
- Bilingual English/Spanish preferred.

Salary
All wages are based on relevant years of experience and/or time since completing a relevant degree in the related fields. The minimum wage is the wage for an individual with no relevant experience, as determined by the job description and prior organizational practices.
