Summary
The Specialized Services Case Manager coordinates behavioral health and benefit services for residents of Nursing Facilities for Mental Health (NFMH) through CrossWinds Counseling & Wellness. This role ensures timely client intake, assessment, and treatment planning, reviews PASRR determinations and continued stay screens, assists with Medicaid and SSI/SSDI navigation, and connects clients to specialized programs such as SOAR, IPS, Housing, and Peer Support.
Essential Functions

- Complete intake assessments with new NFMH residents within 14 business days of admission to ensure enrollment as CMHC clients. For residents already in placement 30+ days, complete intake within 120 business days following the effective date of the agreement.
- Partner with QMHPs, Olmstead Navigators, NFMH staff, and county liaisons to coordinate treatment planning, continued stay, and discharge.
- Review PASRR determination letters, Level II evaluations, and Screens for Continued Stay (SCS) to inform service delivery and benefit eligibility.
- Ensure timely completion and coordination of screens for continued stay.
- Provide targeted benefit navigation including SSI/SSDI, Medicaid, and insurance enrollment; assist with entitlement reviews and redeterminations.
- Assess appropriateness for referrals to specialized services (SOAR, IPS, Housing, Peer Support, Vocational Supports).
- Document resident refusals of Specialized Services and reassess interest regularly in coordination with guardians and staff.
- Maintain complete and accurate records of all client services in compliance with state licensing agencies, funding requirements, and CrossWinds standards. Prepare and submit daily progress notes, client status reports, treatment plan updates, and related documentation in a timely and professional format.
- Collaborate with the Accounting Department to complete monthly reporting and invoicing. Ensure all required Excel templates are accurately submitted by the 20th of each month.
- Develop and maintain up-to-date knowledge of community resources and service providers to effectively connect clients with additional supports and opportunities.
- Establish and sustain positive working relationships with clients, agency staff, external partners, and community stakeholders while maintaining a professional and supportive demeanor.
Skills & Abilities
Education & Experience
Associate’s degree plus two years’ experience in the health and human services field OR four years’ experience in the health and human services field is required. Bachelor’s degree in psychology, social work, or counseling is preferred.
Certifications & Licenses
Must complete Case Management Training and provide certificate within 6 months of hire.
Computer Skills
Computer experience in Microsoft Office programs and basic computer fluency.
Additional Qualifications
Ability to work with persons with severe and persistent mental illness; ability to work independently and as a member of a therapeutic team; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations.
Work Environment
This job operates in both a community and a professional office setting. This position primarily works with clients out in the community, in their homes, in offices of other providers, and within the residential facilities. This role uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.
