Chinook Place, our beautiful community located in Madras, OR has a current opening for a Licensed Practical Nurse. This position will travel between our Central Oregon communities based on different needs. Responsibilities:

Coordinate and provide nursing care and services including:

• Complete resident health assessments
• Monitoring of resident overall health
• Communication with physicians and outside providers
• Investigation of injuries involving residents or staff
• Tracking and auditing of medication and treatment orders
• Part of service planning team
• Coordination of resident care
• Participate in care conferences
• Participate and direct quality assurance investigations
• Maintain compliance with regulatory requirements
• Participate in in-service presentations and staff meetings
• Communicate with state and assist in surveys
• Help foster a positive team attitude in the community

Requirements

Requirements:

• Degree in Nursing and a current state license
• Understanding of state and federal regulations
• Good computer skills
• Ability to lift, push or pull 30 pounds, up to 30 times per shift
• Ability to bend, twist, stoop, kneel, and reach
• Commitment to the safe practices, infection control, residents’ rights
• Strong communication skills and congenial personality
