What You Will Do
- Provides advanced care planning and facilitates Goals of Care discussions with VNS Health members.
- Collaborates with the interdisciplinary team to identify potential Hospice and Palliative Care needs.
- Reviews VNS Health patient records for readmissions occurring during active care episodes.
- Participates in root cause analyses of readmission events in collaboration with VNS Health operations; contributes to the development of quality improvement recommendations.
- Identifies barriers to Hospice and Palliative Care; communicates findings and recommends appropriate interventions to clinical operations.
- Participates in internal and external clinical case review meetings for select patient populations.
- Maintains strong communication and collaborative relationships across care teams to ensure continuity and quality of care.
Licenses and Certifications:
- License and current registration to practice as a Registered Professional Nurse in NYS required
- Certified Case Manager - Commission for Case Manager Certification within 1 year of employment required
- Hospice and Palliative Care certification (CHPN) a plus
Education:
- Associate's Degree in nursing required
- Bachelor's Degree in nursing preferred
Work Experience:
- Minimum two years of experience in health related field required
- Care management and/or managed care experience preferred
- Proficiency in Microsoft Office applications required
- Demonstrated analytical skills required
- Registered Nurse (RN), Nurse Practitioner (NP), or other licensed clinical professional with relevant experience.
- Minimum of 3–5 years of clinical experience in care management, hospice, palliative care, or related fields.
- Strong knowledge of advanced care planning, Goals of Care discussions, and end-of-life care principles.
- Experience with clinical documentation review and quality improvement initiatives.
- Excellent communication, collaboration, and interpersonal skills.
- Ability to work independently and effectively within interdisciplinary teams.
