JOB SUMMARY

The Palliative Care Program APRN is an Advanced Practice Nurse who participates in the development, implementation, and organization of the program. The Palliative Care Program was developed to deliver palliative care and other related health care services to patients and families. The Palliative Care Program APRN will coordinate the continuum of patient care between the attending physician, the patient, and the Medical Center. The APRN will measure outcomes, progress of the program, and identify areas for improvement. The APRN reports to the Director of Case Management. Evaluation of delegated medical acts will be performed by the Palliative Care Program Medical Director.

The Palliative Care Program APRN is responsible for an evidence based program that includes, but is not limited to, patient flow, patient outcomes, patient and family satisfaction, and the following responsibilities: 1.Performs bedside palliative care patient/family evaluation 2.Assists with diagnosis, treatment and management of acute and chronic health conditions. 3.Orders, performs, and interprets laboratory and radiologic tests within the scope of professional practice. 4.Facilitates the development of the Palliative Care Program and coordinates the discharge of selected patients in collaboration with the healthcare team. 5.Develops and keeps up-to-date standardize care plans, protocols, and order sets related to palliative care. 6.Prescribes medications, including controlled substances, to the extent delegated and licensed. 7.Participates in the creation of patient, family, and community education, including the coordination of teaching materials, within the Medical Center. 8.Participates in processes for tracking outcomes and patient satisfaction, and identifying areas that require improvement. 9.Orders treatments and durable medical equipment, as indicated. 10.Provides training and education for staff and medical staff. 11.Consults with Palliative Care Director or designee(s), as needed, informs attending physician of services provided, and collaborates with other physicians, as needed. 12.Prepares and maintains accurate patient records, charts, and documents to support sound medical practice. 13.Complies with applicable laws and regulations with respect to collaborative agreements. 14.Assists in all facets of care coordination for palliative care patients. 15.Assists in development of clinical practice guidelines/standards in support of quality palliative care. 16.Participates in the assessment and treatment of patients in the outpatient Palliative Care Clinic and in skilled nursing facilities after orders are received for a consultation. 17.Other duties, as required.

JOB DETAILS AND REQUIREMENTS

Saint Francis's Hospice and Palliative Care Program is seeking a PRN APRN.

Position Details:

-Join a group of 1 Hospice/Palliative Care physician and 2 employed APRNs

-EMR: Epic

Education:

-Master's degree in Nursing, required

Certification & Licensures:

-Advanced Practice Nurse in Missouri

-BLS certification

Experience:

-Clinical background as a practicing advanced practice nurse.

-Previous experience with chronic diseases is preferred. 

-Experience with patient/staff education.
