Position Function Summary 


Under the direction of the CMO, the Nurse Practitioner provides primary medical and preventive care to patients with acute and chronic illnesses in a n assigned clinic site(s), in accordance with federal, state, and other administrative requirements.


Essential Responsibilities  

  • Provides patient care by examining and diagnosing patients, and carrying out or supervising medical treatment, therapy, and follow-up; referring patients for specialty consultation and treatment; educating patients in primary medical care; documenting patient-doctor transactions for proper record keeping and billing.
  • Assists and receives direction in the provision of patient care by consulting with LFH physicans on the following situations, as needed:
  • When required by protocol.
  • When faced with a situation which may require hospital admission.
  • When incumbent is medically uncertain in a particular clinical situation.
  • When the clinical situation is beyond the scope of practice of the incumbent.
  • Conducts physical exams for all age groups to include health histories, labs, and other tests as appropriate with a focus on prevention.
  • Diagnoses and treats injuries and illnesses, both chronic and severe.
  • Prescribes medications, orders refills and maintains all required pharmacy logs.
  • Orders laboratory tests, interprets results and communicates results to patients.
  • Consults with physicians and other medical specialists and refers patients to an appropriate provider.
  • Performs specialized procedures to include suturing, biopsies, and nail removals.
  • Performs specialized edams to include women’s health exams.
  • Charts and documents all patient interactions appropriately in the electronic medical record, responds to messages and request from nursing staff and patients, conduct chart audits for clinic, completes charting in required timeframe and conforms to meaningful use and other guidelines as required.
  • Participates in immunization clinics, well child clinics, diabetes screenings, and health promotion.
  • Participates in education and training of nurses and medical assistants to improve their clinical skills.
  • Provides support in the overall LFH operation by acknowledging, adhering to, and implementing overall organizational and medical policies and operating procedures of LFH.
  • Provides counseling to parents and/or children, focusing on preventive healthcare; educates patients by disseminating of patient information and literature relating to health education including but not limited to family planning, pre-natal care, and health promotion/disease prevention for all age categories.
  • Maintains compliance of clinic with state licensure by enforcing Quality Assurance protocols in the clinical, pharmacy, laboratory, medical supply, and nursing/patient care areas of LFH for quality patient care.
  • Maintains and updates professional knowledge and skills, and maintains clinical proficiency by attending continuing education or seminars related to job responsibilities, including Age Specific training (care giving, treatment, and assessments specific to the age of populations served).
  • Coordinates with CMO to request their advice and assistance as required, to confer on problems; to provide advice, assistance and information on the provision of patient care, quality improvement initiatives, and other areas under this area of responsibility.
  • Coordinates with other providers, as a member of the primary care team, to improve their clinic practice and patient care skills.
  • Coordinates with other employees, providing leadership as a key member of the primary care team; maintains harmonious, courteous, and understanding relationships, while fostering a collaborative team work environment. Exchanges information and job experiences that would be mutually beneficial to each functional area within LFH.
  • Coordinates with Christus St. Vincent (CSV) Regional Medical Center, Public Agencies, Community Leaders an Organizations, Other Health Care Providers and Organizations including, private physicians, local hospitals, EMS providers, public health department, and other community health centers, professional groups and associations, etc., and Legal and Regulatory Agencies to build and maintain positive relationship with, and to promote the mission and purpose of LFH.
  • Coordinate with Members, Service Area/General Public to provide helpful, courteous assistance and service to LFH members and other customers/constituents, and presents a friendly, courteous image for LFH.
  • Follows Customer Service standards to ensure quality of service.
  • Performs other duties as stated in physician union collective bargaining agreement (CBA).

Requirements

Education, Experience and Skills/Training

  • Graduate of an accredited certified nurse practitioner program. Experience in a community health center, and working within a managed care environment, preferred.
  • Documentation of education, experience and licensure is required and must be maintained. Certified Nurse Practitioner must be insurable under the policies and requirements of LFH.
  • Current licensure for the state of New Mexico, required.
  • Certified in specified area of medical specialty by American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Federal DEA certification, required.
  • NM State pharmacy license, required.
  • ACLS or CPR and First Aid certification, required.
  • Must be credentialed per LFH procedures.
  • Knowledge and skills in patient education and health promotion, diagnosis and treatment, and time management.
  • Knowledge of the concepts, principles, and practices of family medicine.
  • Knowledge of the statutes, regulations, guidelines, and other requirements governing the provision of health care in the State of New Mexico.
  • Ability to resolve patient disputes and conflicts in collaboration with other staff.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, procedure manuals, accreditation standards and guidelines, government regulations, and contracts
  • Ability to write at a high level of competence, including internal and external program reports, correspondence, program planning documents, and proposals.
  • Ability to communicate with diverse groups of people to include staff and providers, governing and advisory boards, patients, government agencies, and independent auditors on behalf of the organization.
