EDUCATION, EXPERIENCE ANTRAINING: 

Current Indiana license as a Registered Nurse required. BSN required. BLS or CPR for Healthcare Providers certification required. Certification from National Certification Board for Diabetes Care and Education (NCBDE) as a Certified Diabetes Care and Education Specialist (CDCES) required. Insulin Pump Trainer Certification preferred.

JOB RESPONSIBILITIES:

Assesses patients referred to the Diabetes Care Center for educational and self-care needs and educates as appropriate for the individual's needs. Documents education according to established guidelines; enters charges for services provided.

Directly participates in the screening, assessment, reassessment, and education process for patients across the continuum of care. Provides quality diabetes self-management education in individual and group settings based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes. Addresses patient telephone questions.

Coordinates and participates in the development and implementation of curriculum, documentation forms and program performance improvement guidelines based on the ADCES 7 self-care behaviors. Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardio metabolic conditions, and self-management of diabetes into clinical practice. Documents education according to established Ensures Association of Diabetes Care and Education Specialist (ADCES) program recognition guidelines.

Provides education programs for physicians/providers, healthcare personnel, and consumer groups as requested and consistent with regulatory and Association of Diabetes Care and Education Specialist (ADCES) program recognition criteria. Serves as a clinical preceptor for nursing and ancillary healthcare students.

Monitors staff schedules to assure patient needs are met within established parameters. Assists in the development and implementation of staff orientation and competency evaluation. Assists in the hiring of Diabetes Care Center staff; evaluates staff work performance and makes recommendations for personnel actions. Designs, implements, and monitors work routines to ensure compliance with departmental policies and procedures, as well as other licensing and accreditation agencies.

Serves as coordinator for the DCC Advisory Committee (Care Council) and participates in Care Council decisions. Serves as the coordinator and leader of the diabetes community classes. Actively participates in the quality improvement process and adapts practice/process based on QI findings.

Performs other related duties as assigned.
