About the Department

The Case Worker – Support and Service Coordinator performs initial assessment and case management of families, adults, and/or children seeking or receiving services from the Department of Human Services (DHS).


Position Duties

  • Works to protect vulnerable adults and children, preserve families, or to keep out-of-home placement at a minimum; strives to achieve active involvement and participation of individuals, family, or significant others involved in the service program.

  • Demonstrates motivational leadership to achieve services and treatment objectives without duplication of effort and with maximum effective use of staff and resources.

  • Identifies full range of client needs through thorough investigation and assessment; differentiates between medical, psychological, vocational, social, familial, daily living spheres; collaborates with children, youth, and families to assess and develop goal oriented, time limited, strength-based service plans to address identified needs.

  • Maintains awareness of resources and support services available; explains the scope of the agency’s services with clients; discusses the client’s rights and responsibilities in relation to the use of such services; educates individuals and familiesabout available community resources and how to independently gain access to needed services; secures, coordinates, and monitors access to services.

  • Demonstrates successful case management utilizing best practices or evidence-based practices; conducts regular case evaluations; develops and updates service plans accordingly; monitors cases to ensure that services are provided in an appropriate and timely manner; reviews progressterminates services when stated goals are reached; maintains case records containing pertinent, accurate, and current information. 

  • Adheres to strict record-keeping and reporting guidelines and laws; fully completes required documentation in timely fashion and in accordance with program requirementsmaintains confidentiality of client-related informationcompletes required written assessments, reports, and other related paperwork, as necessary; prepares correspondence, reports, and other records as required.

  • Establishes and maintains effective working relationships with internal and external partners; serves as advocate for clients within the community; speaks to the community and other organizations to provide public education; attends and participates in staff meetings, supervision times, and other regularly scheduled meetings.

  • Develops professional knowledge and skills by attending training events, conferences, and workshops.

  • Performs other related duties as required or assigned.

Minimum Qualifications

  • Bachelor’s degree in social work or a related field.
  • Appointment will be conditional upon successful completion of criminal and caregiver background checks.

  • Must have valid driver’s license, an acceptable driving record, and access to private transportation for work-related duties

  • Must possess and maintain personal automobile insurance in the amount of $100,000/$300,000.


Other Qualifications

  • Six (6) months of experience in family, children, or adult case management services, or successful completion of an internship serving children, youth, or families.
