Position Title: Lead Community Support Coach


Department: Adult Day Services

 

Reports to: Program Manager


Status: Full-time, Non-Exempt


Hours: Monday - Friday 8AM - 4PM


Summary
To provide support in case-management, behavior management and medication administration for assigned program, to monitor and facilitate ongoing achievement of required reports and trainings, to assist with materials management and program resupply. This position provides material and data support for program staff; maintains individual records, monitors individual data collection; monitors program staff data collection; writes reports, data summaries and updates; assists with progress reporting for Individualized Support Plan (ISP) and other case management instruments; and assists staff and administrators with general program needs.

Requirements

Staff Training and Coverage

  • Coordinates and delivers, as authorized, initial and ongoing staff training, in conjunction with Program Manager and Director of Adult Services
  • Trains new and current staff to competency and provides feedback in the delivery of general supports and in the person-centered supports detailed within an individual’s Plan for Supports (Part 5 Plan for Supports). 
  • Demonstrates training ability that meets or exceeds both organization-wide standards of quality and state code and agency regulatory requirements.
  • Completes and maintains all required trainings including but not limited to: DBHDS Competencies, CPR/First Aid, CDeT, Medication Administration, and Risk Management.


Provides direct service as needed for coverage, including:

  • Assisting persons served to increase their independence and to exercise their rights by teaching skills in various activities of daily living (self-care, care of the household, communication, leisure, etc.) according to formal plans and as natural teaching opportunities arise, using informal teaching techniques/procedures
  • Assisting persons served to reduce maladaptive behaviors and to learn appropriate replacement behaviors by implementing approved behavior management procedures, according to formal plans and as natural teaching opportunities arise
  • Assisting in maintaining the health, hygiene and grooming of persons served by teaching new skills, implementing first aid as needed, health care and hygiene techniques according to instructions or plans and, if properly certified, by administering medications according to stated policies and procedures
  • Providing active treatment by continually engaging persons served in age-appropriate, functional activities, planning and implementing community access activities, and implementing programs to achieve ISP outcomes
  • Transporting clients served as needed to community activities, following Faison vehicle training and using Faison-provided vehicles


Case Management & Administrative Support

  • Assists Program Manager and Director with completing assessments, recommending treatment goals and outcomes, and compiling and summarizing data.
  • Generates and submits reports and progress summaries in a timely manner as indicated by each individual’s ISP timeline, state regulatory requirements and/or as directed by Program Manager or Director of Adult Services.
  • Participates in ISP meetings and team meetings.
  • Schedules staff to individuals, activities, and events according to available daily resources. 
  • Ensures that the daily operations of Faison’s Adult Day program are uninterrupted, especially in the absence of the Program Manager.
  • Risk-management: Closely monitors, identifies, prevents, mitigates and / or responds appropriately to possible risks to health, safety and property including but not limited to: environmental risks, weather related risks, privacy and data security, risks related to use of vehicles, communicable illnesses (e.g., influenza, norovirus, etc.), and medication administration.


Quality Assurance

  • Monitors, collects data and reports on the activities related to the organization’s Quality Assurance/ Improvement Goals.
  • Completes both random and scheduled audits of at least two Adult Day individuals’ records per month.
  • Conducts regular inspections for compliance with both state agency regulations and Faison’s policies and procedures.
  • Documents any issues arising from audit reviews and regular inspections; identifies and implements steps to correct them.

Experience Requirements

  • Knowledge of educational reports 
  • Knowledge of adult services regulations and requirements
  • Ability to interpret and utilize data to generate reports
  • Ability to understand and utilize external information to update reports
  • Ability to interact with program staff professionally
  • Previous experience with data collection preferred

Education Requirements

  • High school degree or equivalent required.
  • Bachelor’s degree in human services or related field preferred


Essential Physical Requirements

· Lift 50 pounds (and keep back straight while doing so)

· Bend at all joints fluently

· Move in and out of and maintain a kneeling position for up to five minutes

· Reach in all directions

· Grasp using both hands  

· Move quickly in both confined and open spaces to avoid safety issues

· Run in order to retrieve an individual who is moving quickly away from staff 

· Maintain balance while lowering to a half squat position

· Ability to hear, see, and speak.
