As a CMS Case Manager within the North Georgia Health District at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting the lives of Georgian residents in a variety of ways.
Location - The North Georgia Health District in Dalton, GA.
Position Duties
Under supervision, provides persons, families or vulnerable populations with the psychosocial support needed to cope with chronic, acute or terminal illnesses and physical or developmental disabilities. Provides patient education and counseling, advises caregivers, and makes necessary referrals for other social services.
- Assures Children's Medical Services program operations include the accurate processing of paperwork, documentation, and record keeping of protected health information.
- Acts as an advocate in order to coordinate required services.
- Monitor, evaluates, and records progress according to measurable goals described in the treatment/care plan.
- Provides care coordination, develops plans of care, educates patient and family, and refers patient to agencies that can assist in meeting the health and/or social needs.
- Documents pertinent observations and responses to treatment.
- Conducts home visits to complete comprehensive assessments of client care needs.
- Assists with monthly orthopedic and quarterly Genetics Telehealth Clinics.
- Facilitates physician/patient communication and treatment via telehealth.
- Assists six counties in District 1-2 in the promotion and enrollment of eligible into Children's Medical Services and Children First Program.
- Develops and sustains community partnerships.
- participates in program outreach.
- Conducts presentations at schools, churches, and organizations within the community.
- May serve on community committees.
- Works closely with other programs within the CwSN unit, such as Children First, Early Hearing Detection & Intervention, and Babies Can't Wait to ensure linkage to all services available.
- Screens and refers clients to other community providers.
- Refers clients or families to community resources and provides access to services such as financial assistance, legal aid, housing, job placement, or education.
- Participates in drills and exercises on mass dispensing.
- Assists in the DPH response to pandemics.
- Performs job duties with minimal supervision.
- Receives additional training to gain full proficiency and experience in all areas, as required.
- Other duties as assigned by supervisor.
Minimum Qualifications
Bachelor's degree in a related field from an accredited college or university AND two (2) years of experience in social service delivery.
Preferred Qualifications
- Experience in providing case management services.
- Bilingual in English and Spanish.
- Public Health experience.

Current State employees are subject to State Personnel Board rules regarding salary.
CBH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered.
