The schedule for this role is Monday through Friday 8:00am-5:00pm!

This role requires flexibility to attend occasional outreach events or meetings as needed!

Purpose and Impact:  

The Project Coordinator is responsible for the implementation and execution of the grant objectives and serves as the primary point of contact for community agencies and stakeholders.  The position will implement projects activities, coordinate monthly meetings, and data collection and sharing among partner agencies involved in the grant. This is a hybrid position expected to work consistent hours weekly in the Jacksonville office.

This position is eligible for remote or hybrid work at management’s discretion; however, candidates must have a reliable internet connection and a dedicated workspace. The company reserves the right to modify the work arrangement at any time based on business needs. Employees may be required to attend occasional in-person meetings or events and must reside in a location where the company is legally authorized to operate and conduct business. Time zone alignment and work hour expectations may apply depending on the team's location.

Essential Functions:

  • Ensure oversight and execution of grant objectives, mandatory project deadlines and care coordination activities in collaboration with grant partners and community stakeholders.
  • Analyze sustainability opportunities and develop plans for program continuation or expansion beyond the grant period.
  • Ensure fidelity to evidence-based models as required by the grant.
  • Oversee the quality management process related to grants, including coordination with external evaluators and use of data to inform decision-making.
  • Lead learning collaboratives to promote the dissemination of best practices and innovations across the provider network.
  • Identify and cultivate relationships with local organizations, corporations, schools and other relevant stakeholders to contribute to the successful implementation of grant-funded initiatives and outcomes.
  • Facilitate coordination among LSFHS departments (e.g., Clinical, Finance, Network Management, Quality) to align internal efforts with grant objectives.
  • Serve as liaison between among community providers, contracted network service providers, system partners, and funders.
  • Coordinate, communicate, and promote data sharing among community stakeholders and agencies according to approved protocols.
  • Develop and coordinate formal partnerships with legally binding agreements among agencies to provide and coordinate services.
  • Monitor progress in meeting performance measures and deliverables; identify gaps in care using data and implement strategies for improvement.
  • Work to remove barriers to access to care and effectively link participants to the most appropriate services in a timely manner. Activities will include data surveillance, information sharing across regional and system partners, and development of partnerships with community stakeholders to facilitate linkage to services and to primary care providers, housing, and other needed supports.
  • Support training on data collection tools, assessments, and instruments used in the grants.
  • Use data to identify trends, monitor retention in care, and drive innovative service delivery.
  • Assist in planning and delivering training, including Train-the-Trainer workshops, evidence-based trainings and stakeholder education in alignment with grant goals.
  • Responsible for the submission of required reports to DCF and stakeholders.

Physical Requirements:

  • Ability to sit for extended periods while working at a computer, attending meetings, and completing documentation.
  • Frequent use of hands and fingers for typing, writing, and handling office equipment.
  • Visual acuity sufficient to read standard text on a computer screen, reports, and documents.
  • Occasional standing, walking, bending, or reaching to access files, supplies, or office equipment.
  • Ability to lift and carry materials, files, or equipment weighing up to 20 pounds.
  • Capacity to communicate clearly and effectively in person, by phone, and through written correspondence.
  • Ability to travel locally between program sites, partner agencies, and meeting locations as needed.
  • Ability to travel within work region and occasionally to LSF Health Systems headquarters in Jacksonville, FL.
  • Valid Florida driver’s license and current insurance.

Education and Experience:

Bachelor’s degree in health or social services, behavioral health or related field with a minimum of three years of experience working in social services/ behavioral health setting required. Master’s Degree preferred.

Skills:

  • Proven ability to analyze and interpret data. 
  • Knowledge of Evidence Based Interventions.
  •  Experience with providing both mental health and substance abuse services to vulnerable populations. 
  • Proven ability in the areas of program coordination, contracts and delivering training and technical assistance programs.
  • Aptitude for prioritizing responsibilities and meeting critical deadlines.
  • Ability to work both independently and collaboratively.
  • Demonstrated ability to communicate effectively, constructively and supportively with staff, providers and community stakeholders while also maintaining high standards of excellence. 
  • Experience in Microsoft Word, Excel and Power Point and Gmail. 
  • Strong written communication skills and demonstrated ability to produce professional documents and reports.
  • Understanding of principles of quality management including data analysis and data driven decision-making, using data to measure and manage program outcomes, and using data to tell the program story.

Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.

Principal Accountabilities:

  • Reports to the Grant Performance & Impact Manager
  • Team player with providers and co-workers.
  • Accurate, complete and timely submission of all required financial assignments.
