HJ Staffing is seeking a compassionate and skilled Behavioral Health Case Manager / Social Worker to provide professional social work services in a psychiatric outpatient setting. This role involves overseeing and participating in the care of individuals with developmental disabilities, mental illness, or emotional disturbances, helping them achieve social, economic, emotional, and physical balance and independence.

The Case Manager will deliver professional behavioral health services including assessments, treatment planning, direct service provision, person-centered interventions, and management of client risk and safety. The position reports to an administrative superior and ensures the highest standards of care in support of the Department of Health and Human Services – Division of Mental Health and Substance Abuse.

Essential Responsibilities

  • Provide comprehensive case management services, including client assessments, treatment planning, monitoring, and follow-up.

  • Deliver direct clinical services, including counseling and therapeutic support.

  • Manage client risk and safety while promoting person-centered recovery and independence.

  • Maintain and update electronic medical records (EMR) with accurate documentation of assessments, treatment plans, progress notes, and outcomes.

  • Prepare and submit narrative reports, case documentation, and compliance records.

  • Provide consultative services to state, local, and private agencies; make referrals to appropriate community resources as needed.

  • Collaborate with interdisciplinary treatment teams to ensure coordinated and effective care.

  • Maintain current knowledge of community resources, behavioral health best practices, and regulatory compliance.

Minimum Qualifications

Candidates must meet at least one of the following qualifications:

  1. Education: Possession of a Master’s Degree in Behavioral or Social Science, or a closely related field.

  2. Case Management Experience: Demonstrated experience in assessing, planning, developing, implementing, monitoring, and evaluating services to meet individual human service needs using counseling/interviewing techniques, service plans, or case management systems.

  3. Clinical Experience: Experience providing clinical treatment, counseling, or determining eligibility for services/benefits as part of a treatment team.

  4. Technical Experience: Proficiency with Electronic Medical Records (EMR).

  5. Documentation Skills: Experience preparing narrative reports and case documentation.

Required Knowledge, Skills, and Abilities

  • Strong knowledge of behavioral health principles, practices, and treatment modalities.

  • Ability to conduct comprehensive assessments and develop person-centered treatment plans.

  • Excellent oral and written communication skills, including case documentation and report writing.

  • Familiarity with community resources and social service networks.

  • Ability to work both independently and collaboratively within a team.

  • Commitment to cultural competence, confidentiality, and ethical standards of practice.
