Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site mission-driven non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program.
GLFHC is currently seek a Prenatal Coordinator. Under the direction of the Maternal Child Health Nurse Manager, the Prenatal Coordinator “Coordinator” is an administrative support role. The individual is responsible for ensuring all prenatal patients whose obstetrical care is being managed by a Greater Lawrence Family Health Center (GLFHC) clinician are receiving timely and accessible care, based on established guidelines. The coordinator serves as a critical interface between the patient and their integrated care team, primarily to schedule first trimester appointments, follow-up on patient inquiries and track the patients longitudinally over the course of their pregnancy. The coordinator is a vital interface between the patient and health care team and assists in coordinating scheduled appoints and diagnostic follow-up for patients who require additional navigation services. The coordinator approaches patients with professionalism and compassion and maintains the highest customer service standards.
- Manages the GLFHC prenatal registry and initiates patient outreach to schedule appointments.
- Completes the SDOH screen with patients in advance of their first trimester nursing appointment.
- Collaborates with the CSS CHW team as needed to address acute needs while providing resources based on screening outcomes.
- Ensures all patients receive their first trimester appointment with the nurse and provider timely and escalates if there are barriers to achieving this goal.
- Tracks patients longitudinally to ensure they have met their appointment and diagnostic milestones and escalates any barriers to the PCP.
- Manages the prenatal phone line and follows up on all patient inquiries.
- Assists with scheduling group visits and helps patients secure access to GLFHC and community support groups.
- Proactively manages the outreach list and identifies patient engagement barriers that require focused intervention due to risk status.
- Assists the prenatal team as needed with patient related support functions as assigned and within scope of practice.
- Ensures all prenatal patients received their 2nd trimester fetal survey ultrasounds as per job duties.
- Renews CPR certification every 2 years.
- Remains current in Medical Assistant skills, including completing yearly MA competencies.
Qualifications:
- Bilingual required
- Highly organized who can work independently, but recognize need for escalation
- Problem solver and ability to influence patients to engage in their care
- Ability to manage spreadsheets and data bases
Education:
- 3 years or more MA or equivalent experience in a health care setting
- Certified in CPR
- High school diploma required and additional certification desirable
