Overview:

We are seeking a Clinical Coordinator at our Montage Mason campus, a luxury assisted living and memory care senior living community in Mason, Ohio. In this role, you’ll promote resident wellness by fostering a person-focused approach to care while guiding nurses and caregivers in delivering services that meet best practice standards and regulatory guidelines. This is the perfect opportunity for an LPN who wants to grow into leadership—providing mentorship, coaching, and clinical oversight—while still having the chance to step in and support direct care when needed.

 

 

Status: Full-time

Schedule: Primarily daytime hours with on call flexibility as needed

Location: 5373 Merton Drive, Mason, OH 

Pay: Starting at $30.00/HR, based on experience

Responsibilities:
  • Works from a team approach to guide staff in meeting residents’/patients’ needs.
  • Establishes an excellent rapport with residents/patients and their families.
  • Coordinates staff assignments, and maintains appropriate staffing ratios.
  • Responsible in completing Quarterly and significant change in condition Level of Care Assessments.  
  • Delegates duties to staff members within each individual team member's scope of practice, training, and competency level.
  • Works closely with Director of Nursing and provides clinical back up in the absence of the Director of
  • Follows up on delegated tasks to verify the task was completed.
  • Trains and onboards new staff to Point Click Care.
  • Responsible for managing the clinical staff with orientation, scheduling, and staff development.
  • Complete skills audits with LPN’ and Caregivers.
  • Responds to urgent and emergency needs immediately.
  • Must have the ability and willing to perform all duties encompassed in the Caregiver and the Staff Nurse job descriptions whenever needed.
  • Collaborates with team to complete admission paperwork and follows up with staff to make sure everything is completed.
  • Audits resident charts and records to assure all necessary forms are completed and accurate.
  • Provides timely communication with culinary, dietitian, dietetic technician, and/or therapy staff as needed for weight or diet concerns.
  • Maintains supervision of care provided by Caregivers and Staff Nurses.
  • Maintains staffing ratios according to unit census.
  • Completes monthly work schedules for Staff Nurses and Caregivers
  • Complete daily staffing sheets and provide to front desk each day.
  • Attends Care Conferences as needed.
  • Provides prompt and accurate response to staff, resident, and family inquiries for information while always honoring confidentiality.
  • Reviews team member's documentation and provides guidance, support, and education where needed.
  • Enforces infection control policies and procedures.
  • Enforces proper medication administration and handling procedures according to state and federal regulations.
  • Performs audits to ensure ongoing compliance with policies and procedures as assigned by the director of nursing.
  • Maintains professional knowledge base.
  • Documents pertinent information using established policies/guidelines.
  • Directs injured staff members in incident report completion and appropriate workers’ compensation procedure as appropriate.
  • Maintains confidentiality.
  • Communicates job/resident related questions/concerns, and suggestions through the established chain of command.
  • Provides a safe and clean environment.
  • Abides by the company Always Behaviors each and every day as we serve our clients, colleagues, and community.
  • All other duties as assigned and/or appropriate to the position.
Qualifications:

Education:   Graduate of Ohio Board of Nursing approved nursing program or school of nursing.

Licenses or other required certifications:   Must have current, unrestricted LPN license to practice nursing in State of Ohio, hold current CPR card.

Experience:  Experience working with an electronic health record system such as Point Click Care (PCC).  Previous experience in long term/geriatrics is preferred. Knowledge of dementia care is preferred.

Specialized knowledge, skills, or abilities:  Familiar with infection control practices, OSHA guidelines relative to Residential Care Facilities, and Ohio Department of Health Regulations.
