OBJECTIVE/PURPOSE:
Under the directions of the Group Home Manager, the Direct Support Professional (DSP) - Residential, Level II provides direct service to individuals, implements Individual Service Agreements (ISA) and participates in the dayto-day activities of the residents. In addition, ensures that program criteria are met.
MAJOR RESPONSIBILITIES:
1.0 DIRECT SERVICE:
Consistently implements each individual's Treatment Plan (Individual Plan of Care or IPC) while fostering independence and maximum growth. Participates in all daily routines including, but not limited to, daily living, self-care, money management, dining, problem solving, interpersonal, leisure time, independent living, and community integration. Performs medication administration, as assigned. Serves as an advocate for individuals. Ensures compliance with regulations, policies, and procedures while implementing ISAs. Delivers consistent approach to ongoing household management, i.e., housekeeping, ordering supplies, maintenance, and vehicle upkeep. Communication with nursing staff for medical needs when appropriate. Ensure medication accuracy.
2.0 PROGRAM IMPLEMENTATION/PROGRAM DEVELOPMENT:
Functions as a member of the interdisciplinary treatment team. Attends meetings as scheduled. Implements IPCs as written. Assists in the development of each individual program of service. Prepares training objectives as assigned. Serves as an advocate for individual clients.
3.0 RECORD KEEPING:
Accurately enters data, progress notes, monthly summaries, and staff information daily. Fully completes incident reports in a timely fashion.
4.0 TEAM LEAD:
Provides direction to staff by ensuring that shift responsibilities are completed, including: consumer programming and services as outlined in IPCs are completed; scheduled appointments are met; and household is maintained in accordance with the Department of Health standards. In the absence of the Group Home Manager, oversees all Group Home operations; ensures that there is adequate staff coverage for any cancellations that occur. Communicates with the Group Home Manager about programmatic issues and household operations. Supports the decisions of the management team by demonstrating a positive attitude. Reports programmatic and household issues to the next shift. In the event of an emergency/crisis, assumes the role of leader, directing actions of staff. Reports emergency/crisis to appropriate management staff. Assists in the orientation and training of new staff in all areas of fire safety and evacuation procedures, household operations, recorded keeping and charting, incident reporting, diets and meal preparation, finances, regulations, and individual specific programs. Helps to schedule Staff Team and Client meetings for trainings.
5.0 HOUSEKEEPING:
Performs general housekeeping activities and cleaning duties as assigned. Promptly reports problems with the condition of the furniture, appliances, house, and vehicle to the Group Home Manager. Completes grocery shopping process as assigned.
* In addition to the above, performs other duties as assigned.
Qualifications
EDUCATIONAL REQUIREDMENTS:
High School Diploma or equivalent. Typing skills, windows skills, both typing and navigating, and e-mail usage.
LICENSES / CERTFICATION REQUIREMENTS:
Valid Driver’s License
