Pay Scale: AP: $19-$20/hr. QP:$23.08-$24.04/hr.
Summary:
Under direct and indirect supervision, provides care management services, person centered planning and documentation, referral and linkage, and monitoring/follow-up for clients referred to Davidson County Case Management program through Davidson County Court System.
- Essential Duties and Responsibilities:
Provides care management services to individuals that are under the supervision of the court including addressing social determinants of health, assisting with referrals to appropriate services and resources, monitoring progress, and working with court system to ensure compliance. - Participates in court activities for referred clients. Attend court and/or meetings with court, probation, parole to provide updates on client’s status and progress.
- All other duties as assigned by supervisor
The responsibilities of the Case Manager include, but are not limited to, the following:
Case Management Services
- Provide care management services to individuals including addressing social determinants of health, assisting with linkage and coordination of referrals to appropriate services and resources
- Monitor client progress and work with court system to ensure compliance
- Provide updates to probation and parole on client’s status and progress
- Davidson County Probation and Parole will make referrals for program following a standardized referral processes
- This program is voluntary and must be agreed upon by the individual
Caseloads:
- There will be an average of thirty (30) persons served annually by each case manager. Number may vary.
Data Management/Reporting
- Case Manager will work with Daymark IT/Data Analytics Team to provide quarterly reporting on the following:
- Number being served
- Number of cases closed (end of 90 day follow-up period)
- Number of individuals returning to the court system while care management services are being utilized for 90 days following complete of care management services
- Number of an individual’s subsequent criminal charges in 90 days after care management services completed
- Job Placement and length of employment (during care management services only)
- Number of workforce development referrals
- Number of other outside agency referrals
Special Attributes:
Strong psychosocial, clinical assessment skills. Minimal supervision regarding use of time, able to prioritize work assignments. Ability to communicate effectively with professions and clients/families. Ability to make sound decisions in emergency situations.

Education and/or Experience:
A minimum of an Associate’s Degree in a human services field is required. Bachelor’s degree in a human services field is preferred. Experience in case management, MH/SU field preferred.
