Job Title: Registered Nurse
Job Summary:
CASES is looking for a Registered Nurse to join the Young Adult Assertive Community Treatment (YA ACT) team. This is a multi-disciplinary team that provides evidence-based, mobile, community-based mental health treatment to promote the recovery and community integration of individuals age 18 to 25 with serious mental illnesses, high rates of co-occurring substance use disorders and high utilization of emergency/crisis services. The team also provide developmentally appropriate support to build and implement productive vocational or education plans and will focus on enhancing necessary skills for independent living and self-management.
The Registered Nurse provides medication management and administration, health assessment, counseling, and psychoeducation to support participant stability and recovery. At least 80% of services are delivered in the community, in alignment with the core principles of ACT, professional nursing ethics, and regulatory standards.
Shift Hours: This position is full-time Monday – Friday from 9:00 AM – 5:00 PM.
Salary: The salary for this role is $100,940 per year.
Location Address: 424 East 147 Street, 2nd Floor, Bronx, N.Y. 10455. Our offices are easily accessible by public transportation.
Workplace Flexibility: Flexible - Staff have the flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.
What You Will Be Doing:
- Provides medication management and health services to consumer in the community, including but not limited to: Medication administration (both oral and intramuscular), monitoring medications side effects and adherent, phlebotomy, coordination of consumers' health care services, providing psychoeducation to consumers about health and medications, assessing consumers' mental statues, rotating on team's after-hour emergency line.
- Complete and signs progress notes, nursing assessments, incident reports, and other required documentation in accordance with the New York State Office of Mental Health (OMH) regulations, the New York City Department Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations.
- Coordinate with YA ACT prescriber on consumers' medication management needs, health care services, maintaining consumers' medication administration schedules.
- Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers.
- Participate in team and clinical meetings to collaborate and discuss clinical issues and clients' needs with other team members.
- Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in the hallway, to the appropriate facilities staff member or supervisor.
- Use motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services.
- Any other duties as required by Team Leader and senior clinical leadership.
What We Are Looking For:
- Registered Nurse (RN) with current New York State license
- At least Three (3) years of experience working with young adults (ages 18-25) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service settings(s) care for individuals experiencing homelessness, justice involvement or limited social support
- Extensive knowledge of community treatment, support services, and resources
- Highly organized and self-motived
- Proficient in computers
Preferred Skills:
- Fluency in Spanish.
