Location: Rogers Park or Chatham
The Front Door Diversion Program is an Illinois State grant funded program designed to ‘divert’ individuals from an inpatient psychiatric hospital to the community in lieu of a Specialized Mental Health Rehabilitation Facility. The program offers an array of services such as community mental health supports through an integrated healthcare model and long-term housing supports through transitional programs or subsidy services.
The Assistant Team Leader is responsible for assisting the Team Leader in managing the day-to-day functions of the team. These duties will vary depending on the needs of the team, but may consist of reviewing paperwork and documentation, team scheduling, providing consultation to staff, training, crisis management, etc. The Assistant Team Leader will maintain a reduced caseload and perform daily duties of a Diversion Specialist. They will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.
Responsibilities:
Clinical Caseload:
- Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
- Assist with initial intake appointments and ensure team meets with new discharges within 24 hours of discharge.
- Coordinate the establishment of an individual recovery plan with the client, client’s support system, and other care providers.
- Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services as indicated.
- Assist clients in identifying signs and symptoms of de-compensation. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced.
- Assist clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items
- Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK
- Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
- Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate
- Communicate effectively within the team model for a multi-disciplinary approach to client care.
- Support clients in identifying and applying to permanent supportive housing.
- Assist team with client crisis management (I.e. conduct 2-person home visits, complete petitions, etc.) as directed
- Cover Maximus referrals on Saturday, including liaising with partner hospitals, confirming patient discharge dates, and completing assessments as needed.
- Provide Saturday coverage to support both Front Door Diversion teams with emergent client needs.
Administrative:
- Serve as the team Supervisor in the absence of the Team Leader.
- Support team leader in running team meeting and run meetings in absence of TL.
- Assist team with scheduling appointments as needed and in prioritizing client needs.
- Assist with monitoring for the completion of required documentation (e.g. IM+CANS, LOCUS). ]
- Assist team leader in communicating with the financial services department to ensure clients have access to BMF or needed petty cash.
- Assist team leader with onboarding, training, and educating new employees on the team to provide quality services to clients
- Assist with the completion of transfer paperwork, schedule transfer meetings, and assist in facilitating transfer meetings.
- Perform other related duties and/or projects as assigned by TL and/or PM
Qualifications
- Must have one of the following requirements:
- Bachelors in a related field, any Bachelors degree with two years of experience
- CRSS certification through IAODAPCA
- High school diploma with 5 years of supervision in field such as counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
- Master's degree in social work, Psychology, or Counseling, or other closely related field preferred
- Valid Illinois driver’s license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
- Experience working with people with mental health and/or substance abuse diagnoses required
